Environmental Health Department <br />AMENDED <br />10/18/2019 <br />Small Quantity Hazardous Waste Generator Inspection Report <br />Date: <br />October 11, 2019 <br /> Facility Address: <br /> 2545 W HAMMER LN, STOCKTON <br /> Facility Name: <br /> SUTTER GOULD MEDICAL FOUNDATION <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />RemarksItem # <br /> 104 CCR 66262.34(d)(2) No modified contingency plan. <br />OBSERVATION: A complete modified contingency plan was not posted at the facility. <br />REGULATION GUIDANCE: The business operator must post the following information next to telephones or in <br />areas directly involved in the generation and accumulation of hazardous waste: the name and emergency <br />telephone number of the emergency coordinator, the location of fire extinguishers and spill control material, and the <br />telephone number of the fire department unless the facility has a direct alarm. <br />CORRECTIVE ACTION: Immediately post the required information at the facility. Provide a statement to the San <br />Joaquin County Environmental Health Department that the correction is complete. <br />This is a minor violation. <br /> 105 CCR 66262.34(d)(2) No emergency coordinator. <br />OBSERVATION: A complete modified contingency plan with an assigned emergency coordinator was not posted at <br />the facility. <br />REGULATION GUIDANCE: The emergency coordinator or his designee must respond to any emergencies that <br />arise. The business operator must post the following information next to telephones or in areas directly involved in <br />the generation and accumulation of hazardous waste: the name and emergency telephone number of the <br />emergency coordinator, the location of fire extinguishers and spill control material, and the telephone number of the <br />fire department unless the facility has a direct alarm. <br />CORRECTIVE ACTION: Immediately post the required information at the facility. Provide a statement to the San <br />Joaquin County Environmental Health Department that the correction is complete. <br />This is a minor violation. <br /> 403 CCR 66262.34(d)(2) Failed to keep hazardous waste containers closed except when adding or removing hazardous <br />waste. <br />OBSERVATION: The following open hazardous waste storage containers were observed on-site: <br />- one approximately 15 gallon RCRA Hazardous Waste container located in orthopedics room 1108 was open <br />- one approximately 3 gallon RCRA Hazardous Waste container located in general surgery room 2256 was open <br />REGULATION GUIDANCE: A container holding hazardous waste shall always be closed during transfer and <br />storage, except when it is necessary to add or remove waste. <br />CORRECTIVE ACTION: Immediately close these containers and ensure all hazardous waste containers are closed <br />when not adding or removing waste.
