The containers <br />were: <br />- one approximately 15 gallon RCRA Hazardous Waste container containing pathology specimen bottles in urology <br />room 3219 was lacking generator name and address, hazardous properties, composition, and physical state <br />- one approximately 15 gallon RCRA Hazardous Waste container with cast waste in orthopedics room 1108 was <br />lacking generator name and address, hazardous properties, composition <br />- one approximately 3 gallon RCRA Hazardous Waste container containing hand sanitizer bottles in obstetrics room <br />2041 was lacking composition and generator name and address <br />- one approximately 15 gallon RCRA Hazardous Waste container containing pathology specimen bottles in urology <br />room 3219 was lacking generator name and address, hazardous properties, composition, and physical state <br />- one approximately 1.5 gallon RCRA Hazardous Waste container containing sodium bicarbonate in general surgery <br />room 2256 was lacking composition and generator name and address <br />- one approximately 3 gallon RCRA Hazardous Waste container containing silver nitrate sticks in general surgery <br />room 2256 was lacking generator name and address, composition, hazardous properties, physical state, and <br />accumulation start date <br />REGULATION GUIDANCE: All hazardous waste containers shall be marked with the following information: the <br />words “Hazardous Waste”, name and address of generator, hazardous properties, physical state, composition <br />(contents), accumulation start date <br />CORRECTIVE ACTION: Immediately label the containers as required for hazardous waste. Provide a corrective <br />action statement and supporting documentation to the San Joaquin County Environmental Health Department <br />(EHD) within 30 days. <br />This is a minor violation. <br />Overall Inspection Comments: <br />An inspection checklist was provided to the facility operator on the day of inspection. The EHD has written the <br />complete report on October 18, 2019 which replaces the initial checklist. <br />Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br />documenting the corrective actions that have been or will be taken for each violation, and any supporting <br />paperwork, within 30 days of receiving the complete inspection report. <br />Waste Stream Observed: waste specimen bottles containing formalin, infusion waste, waste sanitizer bottles, <br />silver nitrate sticks, and waste sodium bicarbonate <br />Documents Provided: hazardous waste labeling information, contingency plan template, modified contingency <br />plan template, free CUPA classes flyer, Return to Compliance Certification and Corrective Statements report <br />Note: Sutter Health can combine the facilities at 2505 W. Hammer Lane, 2545 W. Hammer Lane, and 8011 Don <br />Avenue under one hazardous waste small generator program and one hazardous material business plan <br />program.
