According to a work <br />order dated 7/3/18, the "87 fill drop tube flapper is stuck open" and "construction will need to return ... and replace <br />drop tube." No work orders were indicating this flapper was replaced. No permits have been obtained from EHD for <br />flapper/drop tube replacement. The overfill prevention test report dated 9/18/18 states the 87 flapper could not be <br />tested because it was stuck and could not be removed. The designated operator inspection reports state the 87 <br />flapper was tested on 10/26/18, but no work orders or test results were found on site. Immediately repair or replace <br />the failed component by a properly licensed, trained, and certified contractor under permit and inspection of the <br />EHD. If proof that the site has a functioning overfill prevention method in place cannot be provided immediately, <br />there is a possibility that the 87 UST system may be red tagged to prevent fuel inputs. <br />Overfill prevention testing has not been conducted for the 87 flapper. This testing was required to be conducted by <br />October 13, 2018. Immediately schedule this testing and provide 48 hours notification to the EHD. Ensure all future <br />testing is conducted within required time frames. <br />This is a Class II violation. <br />Overall Inspection Comments: <br />Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br />documenting the corrective actions that have been or will be taken for each violation, and any supporting <br />paperwork, by March 10, 2019. <br />"The monitoring system certification was conducted on 2/6/19. The sumps and UDCs were visually inspected <br />by the EHD, but the testing was not witnessed by EHD. The paperwork inspection was conducted today <br />(2/8/19). The technician, Michael Deatherage, did not have proof of certifications for VMI Tanknology Tester or <br />Franklin Fueling Phase I (spill bucket). Jesus Saldivar with Tanknology called on 2/7/19 to notify EHD that he <br />had conducted the leak detector and spill container testing on 2/6/19; his certifications were confirmed. <br />Starting September 1, 2018, all in -office CERS help will be provided at EHD hourly rate ($152).
