Current financial responsibility documents are required <br /> to be submitted annually. Immediately log into the California Environmental Reporting System (CERS) at <br /> http://cers.calepa.ca.gov/, and upload the required documents. <br /> This is a repeat violation, Class II. <br /> 115 CCR 2715(a)(1)(B), 2715(a)(3)"Designated Underground Storage Tank Operator Identification Form" not submitted <br /> within 30 days. <br /> The designated operator identification information submitted in December 2018 is on an outdated form. Any <br /> changes shall be reported to the EHD within 30 days. Immediately log into the California Environmental Reporting <br /> System (CERS) at http://cers.calepa.ca.gov and upload the notification identifying all the designated operators for <br /> this facility. <br /> This is a minor violation. <br /> 118 CCR 2715(a)(1)(A), 2715(a)(2) Failure to submit the "Underground Storage Tank Statement of Understanding and <br /> Compliance Form". <br /> The Underground Storage Tank Statement of Understanding and Compliance Form was not found in CERS. This <br /> form must be uploaded to the California Environmental Reporting System (CERS). Immediately log into the <br /> California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov and upload the Underground <br /> Storage Tank Statement of Understanding and Compliance Form for this facility. <br /> This is a minor violation. <br /> 201 CCR 2712(b) Failure to maintain monitoring, maintenance, and follow up action records. <br /> -Monitoring system certification test results for 2017 and 2016 were not found on site. <br /> -Secondary containment test results were not found on site. <br /> -The alarm history was cleared sometime between 2/28/18 and 3/16/18. No work orders were found on site for this <br /> work. <br /> -Two work orders on site, one dated 8/2/18 and the other undated, state the L13 91 fill sump sensor needed to be <br /> replaced. No work orders were found on site for addressing this issue. <br /> These records shall be maintained on site for at least three years. Monitoring records include: (1)date and time of <br /> all monitoring or sampling; (2) monitoring equipment calibration and maintenance records; (3) results of any visual <br /> observations; (4) results of sample analysis performed a lab or in the field; (5) logs of all readings of gauges or other <br /> monitoring equipment, ground water elevations, or other test results; (6) results of inventory readings and <br /> reconciliations.
