The Facility Employee Training was recorded on an outdated form. Employee training was last conducted on 12/21/18. All site employees must be trained prior to taking responsibility as a facility employee if after October 1, 2018, or within 30 days of hire if prior to October 1, 2018. The Facility Employee Training Certificate shall contain accurate information for all employees trained. Update the Facility Employee Training Certificate to contain the accurate information and submit a copy of the updated form to the EHD. This is a minor violation. 204 CCR 2712(1) Current monitoring plan approved by the EHD not found on site. An approved copy of the monitoring plans were not available for inspection. A copy of these plans shall be accessible on site at all times. Immediately locate and retain a copy on site. This is a repeat violation, Class II. 206 CCR 2712 Failure to comply with UST operating permit conditions. The alarm history was gone prior to March 16, 2018. No paperwork was on site for any work. No notification was received by the EHD. The following conditions are required to meet No Permit Required requirements: 1. Email EHD a scope of work by emailing to UST@sjcehd.com. 2. Notify the EHD of the time and date of the repair work by emailing to UST@sjcehd.com or fax to (209)468-3433. An EHD inspector may stop by the facility to inspect the repair and verify the technician's certifications. 3. The service company must install the part and test the system to verify functionality. 4. The service company must provide the following documentation to the tank owner/operator for inspector review during annual UST inspection: a. Description of part(s) installed b. Name& UST ICC#of Service technician doing the work (manufacturer certification proof) c. Testing done to verify functionality of the repair Immediately obtain a permit from EHD for this work. A'Failure to Obtain a Permit' penalty will apply. This is a repeat violation, Class II. 209 CCR 2716(a-e) Failure to conduct the designated UST operator visual inspection at least once every 30 days. The designated operator failed to check and note all required items on the visual inspection reports. -Tank tightness, and line tightness test information was left blank on the 10/31/18 and 12/21/18 inspection report. -The 10/31/18 report left blank the question about UDC sensor locations. During the visual inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and shall check all required items on the report. Ensure that designated operators performing visual inspections at this facility are including all of the required information on the reports. This is a repeat violation, Class II.
