Medical Waste Tracking Documents <br />Tracking documents, provided by the waste haulers, must be signed by the hospital and waste hauler, <br />indicating release and receipt of the waste, respectively. One copy of the document will be retained by <br />the hospital at the time of shipment and must be maintained in the Safety Binder. Upon receipt at the <br />disposal facility, the tracking document will be completed by the disposal company and can be accessed <br />by the hospital at www sharpstracer.com. Completed tracking documents for each hospital must be <br />maintained for a minimum of three years. <br />Tracking of expired DEA -controlled pharmaceuticals is conducted in accordance with DEA regulations, <br />and documentation can be obtained from Reliable Pharmaceutical Returns. <br />E. Medical Waste Management Emergency Response Plan <br />The following response plan applies to spills of biohazardous and sharps waste only. Any spills of <br />pharmaceuticals or other hazardous wastes (gram stain, isopropyl alcohol, etc.) and any comingled spills <br />(for example, tissue samples in formalin) must be managed as hazardous waste, in accordance with the <br />Hazardous Waste Emergency Response Procedures (Section III.G of the Hazardous Waste Management <br />Section). <br />A spill cleanup kit consists of the following items: <br />® absorbent (cat litter), <br />® scraper and scoop, <br />® personal protective equipment (PPE—water-resistant gown, eye protection, chemical -resistant <br />gloves), <br />® surface disinfectant, <br />® surface wipes, <br />® hand wipes, <br />® instructions, and <br />® biohazard bag or container. <br />7 <br />anfield <br />PET 140 }PITAL <br />
