SA, A O `A�U Itp,1` Environmental Health Department <br /> COUNTY— <br /> Small <br /> OUNTY—Small Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> DOLLAR TREE#07648 318 W MAIN ST RIPON May 15 2019 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 106 CCR 66262.34(d)(2) Failed to train employees on waste handling and emergency procedures. <br /> Based on the types of violations observed,the facility employees are not trained on proper hazardous waste(HW) <br /> handling or the training provided has been inadequate. <br /> -An Aerosols, Flammables, &Combustibles(AF)HW tote with accumulation start date 3/16/19 was not marked with <br /> the physical state(IMG 6333).The AF HW tote contained the following(IMG 6334): <br /> o An aerosol can of Air Wick air freshener <br /> o Two containers of solid air freshener <br /> o Three empty containers of hand sanitizer, marked with"store-use"stickers indicating the containers were used <br /> for their intended purpose by the facility employees. <br /> o A container of hand sanitizer in a silicone holder <br /> o A broken container of nail polish and used paper towels used to clean the spilled nail polish <br /> Renesha Jackson,store manager,marked the AF HW tote with the physical state at the time of inspection(IMG <br /> 6335). <br /> -The following HW items were found in a State-Regulated, Toxics, Electronics, OTC Pharmaceuticals, Universal <br /> Waste(STOU) HW tote with accumulation start date 2/18/19(IMG 6351-5): <br /> o Three empty 32 fl. oz. containers of Nu Veau Hydration Body Wash. Renesha stated she did not know what <br /> happened to the liquid body wash once in the containers, but said that the containers may have arrived to the facility <br /> already empty. <br /> o Three empty 32 fl. oz. containers of Silkience Salon Series Hydrating Shampoo with used paper towels, which <br /> Renesha stated were used to clean-up a spill of the shampoo. The paper towels appeared fairly dry and the amount of <br /> paper towels did not account for the amount of liquid missing from the containers. Renesha did not know what <br /> happened to the rest of the clean-up material or liquid. <br /> -An open bag labeled"Hazardous Waste,"containing used paper towels and a white powdery substance,was found <br /> adjacent to the facility's mop sink(IMG 6363-6). Renesha wasn't sure what the paper towels were used for, but said <br /> the white powder may be Ajax or Comet cleanser which contains bleach. Renesha placed the paper towels in an <br /> Oxidizers, Reactives(OX)HW tote. <br /> -The following was found in a cabinet adjacent to the facility's mop sink(IMG 6367-79): <br /> o A bag labeled"Stericycle Environmental Solutions StrongPak"and"One Waste Per Bag,"which contained used <br /> paper towels and a white powdery substance,which Renesha stated was Ajax or Comet cleanser. <br /> o A bag labeled"Stericycle Environmental Solutions StrongPak"and"One Waste Per Bag"which contained two <br /> sponges, used paper towels, a lid from an Ajax or Comet container, and Ajax or Comet residual. <br /> o A bag labeled"Stericycle Environmental Solutions StrongPak"and"One Waste Per Bag"which contained an <br /> open container of Comet cleanser with bleach and an open container of Ajax cleanser with bleach.When Renesha <br /> scanned the Comet and Ajax,the hand-held scanner stated the cleaners should be placed in an STOU HW tote. <br /> However,according to the Dollar Tree Potentially Hazardous Material Management poster(Poster)posted adjacent <br /> the HW storage, items containing bleach are classified as OX. <br /> o A bag labeled"Stericycle Environmental Solutions StrongPak"and"One Waste Per Bag,"which contained two <br /> sponges and Ajax or Comet residual. <br /> o A bag labeled"Stericycle Environmental Solutions"and"One Waste Per Bag,"which contained a 30 fl.oz. <br /> container of dish soap with a broken cap. <br /> FA0000734 PR0543984 SCO01 05/15/2019 <br /> EHD 22-01 Rev.09/27/2018 Page 6 of 17 Small Quantity Hazardous Waste Generator OIR <br /> 1868 E.
