Based on the types of violations observed,the facility employees are not trained on proper hazardous waste(HW) handling or the training provided has been inadequate. -An Aerosols, Flammables, &Combustibles(AF)HW tote with accumulation start date 3/16/19 was not marked with the physical state(IMG 6333).The AF HW tote contained the following(IMG 6334): o An aerosol can of Air Wick air freshener o Two containers of solid air freshener o Three empty containers of hand sanitizer, marked with"store-use"stickers indicating the containers were used for their intended purpose by the facility employees. o A container of hand sanitizer in a silicone holder o A broken container of nail polish and used paper towels used to clean the spilled nail polish Renesha Jackson,store manager,marked the AF HW tote with the physical state at the time of inspection(IMG 6335). -The following HW items were found in a State-Regulated, Toxics, Electronics, OTC Pharmaceuticals, Universal Waste(STOU) HW tote with accumulation start date 2/18/19(IMG 6351-5): o Three empty 32 fl. oz. containers of Nu Veau Hydration Body Wash. Renesha stated she did not know what happened to the liquid body wash once in the containers, but said that the containers may have arrived to the facility already empty. o Three empty 32 fl. oz. containers of Silkience Salon Series Hydrating Shampoo with used paper towels, which Renesha stated were used to clean-up a spill of the shampoo. The paper towels appeared fairly dry and the amount of paper towels did not account for the amount of liquid missing from the containers. Renesha did not know what happened to the rest of the clean-up material or liquid. -An open bag labeled"Hazardous Waste,"containing used paper towels and a white powdery substance,was found adjacent to the facility's mop sink(IMG 6363-6). Renesha wasn't sure what the paper towels were used for, but said the white powder may be Ajax or Comet cleanser which contains bleach. Renesha placed the paper towels in an Oxidizers, Reactives(OX)HW tote. -The following was found in a cabinet adjacent to the facility's mop sink(IMG 6367-79): o A bag labeled"Stericycle Environmental Solutions StrongPak"and"One Waste Per Bag,"which contained used paper towels and a white powdery substance,which Renesha stated was Ajax or Comet cleanser. o A bag labeled"Stericycle Environmental Solutions StrongPak"and"One Waste Per Bag"which contained two sponges, used paper towels, a lid from an Ajax or Comet container, and Ajax or Comet residual. o A bag labeled"Stericycle Environmental Solutions StrongPak"and"One Waste Per Bag"which contained an open container of Comet cleanser with bleach and an open container of Ajax cleanser with bleach.When Renesha scanned the Comet and Ajax,the hand-held scanner stated the cleaners should be placed in an STOU HW tote. However,according to the Dollar Tree Potentially Hazardous Material Management poster(Poster)posted adjacent the HW storage, items containing bleach are classified as OX. o A bag labeled"Stericycle Environmental Solutions StrongPak"and"One Waste Per Bag,"which contained two sponges and Ajax or Comet residual. o A bag labeled"Stericycle Environmental Solutions"and"One Waste Per Bag,"which contained a 30 fl.oz. container of dish soap with a broken cap.
