The current submitted site map is incomplete. The site map does not address all the required elements. The site map does not address the followings: 1-The locations of the chemicals stored at this facility. The only chemicals locations addressed on the site map are the Waste Accumulation Areas. 2-Chemicals loading areas 3-Emergency Response Equipments(Fire Extinguisher, Eye Wash/Shower stations, and First Aid kits) 4-Emergency Shut Offs Including but not limited to:Ammonia emergency stops/shut offs 5-Access and Exits Points 6-Evacution Areas A site map shall contain a north orientation, loading areas, internal roads, adjacent streets,storm and sewer drains, access and exit points,emergency shutoffs, evacuation staging areas, hazardous material handling and storage areas, and emergency response equipment. If a site map element is not applicable for your facility then list it on the map and label as"NA". Immediately log into the California Environmental Reporting System(CERS)at http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. This is a repeat violation, Class II. 7 HSC 25505(a)(3) Failed to establish and/or submit emergency response procedures for a release or threatened release. The emergency response/contingency plan submitted to CERS contains inaccurate information .The emergency response/contingency plan listed phone number(209)468-4400 in Section C of the Plan as the emergency phone number for SJC EHD. This information is incorrect.This phone number is for SJC Sheriffs Office. The phone number for SJC EHD is(209)468-3420. A business plan shall include the following emergency response procedures for a release or threatened release of hazardous materials, including, but not limited to, the following: -immediate notification of local emergency personnel and the Environmental Health Department(EHD) -procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to persons, property,or the environment -evacuation plans and procedures, including immediate notice, for the site Immediately log into the California Environmental Reporting System(CERS)at http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. This is a Class II violation. Overall Inspection Comments: On-site to conduct a Hazardous Materials Business Plan Inspection. There were three violations during the inspection.An inspection checklist was emailed to Bill Boscacci yesterday. SJC EHD completed and amended the report.The report was amend to add more details and observations for Violation#5.
