The site map does not address all the required elements. <br /> The site map does not address the followings: <br /> 1-The locations of the chemicals stored at this facility. The only chemicals locations addressed on the site map are <br /> the Waste Accumulation Areas. <br /> 2-Chemicals loading areas <br /> 3-Emergency Response Equipments(Fire Extinguisher, Eye Wash/Shower stations, and First Aid kits) <br /> 4-Emergency Shut Offs Including but not limited to:Ammonia emergency stops/shut offs <br /> 5-Access and Exits Points <br /> 6-Evacution Areas <br /> A site map shall contain a north orientation, loading areas, internal roads, adjacent streets,storm and sewer drains, <br /> access and exit points,emergency shutoffs, evacuation staging areas, hazardous material handling and storage <br /> areas, and emergency response equipment. If a site map element is not applicable for your facility then list it on the <br /> map and label as"NA". <br /> Immediately log into the California Environmental Reporting System(CERS)at http://cers.calepa.ca.gov/, upload <br /> the correct or updated information, and submit to the EHD for approval. <br /> This is a repeat violation, Class II. <br /> 7 HSC 25505(a)(3) Failed to establish and/or submit emergency response procedures for a release or threatened <br /> release. <br /> The emergency response/contingency plan submitted to CERS contains inaccurate information .The emergency <br /> response/contingency plan listed phone number(209)468-4400 in Section C of the Plan as the emergency phone <br /> number for SJC EHD. This information is incorrect.This phone number is for SJC Sheriffs Office. The phone <br /> number for SJC EHD is(209)468-3420. <br /> A business plan shall include the following emergency response procedures for a release or threatened release of <br /> hazardous materials, including, but not limited to, the following: <br /> -immediate notification of local emergency personnel and the Environmental Health Department(EHD) <br /> -procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons, property,or the environment <br /> -evacuation plans and procedures, including immediate notice, for the site <br /> Immediately log into the California Environmental Reporting System(CERS)at http://cers.calepa.ca.gov/, upload the <br /> correct or updated information, and submit to the EHD for approval. <br /> This is a Class II violation. <br /> Overall Inspection Comments: <br /> On-site to conduct a Hazardous Materials Business Plan Inspection. There were three violations during the <br /> inspection.An inspection checklist was emailed to Bill Boscacci yesterday. SJC EHD completed and amended <br /> the report.The report was amend to add more details and observations for Violation#5.
