3. The manifests listed below have been provided to the store (EXHIBIT B). The manifests <br />were resubmitted to DTSC by Stericycle on April 26, 2019 with USPS tracking number <br />7018-3090-0002-1854-6903. <br />No. 011385056 FLE <br />No. 011385055 FLE <br />4. Our hazardous waste vendor, Stericycle, submits manifests to the DTSC on behalf of the <br />company and has resubmitted the manifests. Please see the vendor letter of responsibility <br />(EXHIBIT C). <br />5. Our hazardous waste hauler is Stericycle Specialty Waste Solutions, Inc. with US EPA <br />ID no. MNS 000110924. <br />6. The spilled laundry detergent was cleaned up with paper towels, then bagged and re- <br />bagged. The container and the paper towels were also contained within plastics bags. All <br />materials were placed in an STOU tote. <br />7. Containers are labeled completely and correctly. Old labels were removed and proper <br />labels were created (EXHIBIT D). <br />8. Weekly Inspections have resumed (EXHIBIT E). <br />9. Family Dollar store numbers had been converted when acquired by Dollar Tree. The <br />original store number was 12181. The current number is 32181 as shown in DTSC’s <br />system. However, the Family Dollar numbers in CERs are the old numbers. We have <br />begun the process of updating those numbers. <br />The signed certification is also included.
