SAN JOAQUIN Environmental Health Department
C010NTY-
rr Greotness grows herk;. Time In: 11:36 am
Time Out: 12:11 pm
Food Program Complaint Inspection Report
Name of Facility: FAMILY DOLLAR#32078 Date: 02/25/2020
Address: 66 S WILSON WAY, STOCKTON 95205
Owner/Operator: DOLLAR TREE STORES INC Telephone: (757)321-5000
Program Element: 1600-FOOD PROGRAM Complaint#: C00051662
DESCRIPTION OF COMPLAINT ALLEGATIONS
The San Joaquin County Environmental Health Department received the following complaint:
MICE/RAT ALL THROUGHOUT STORE. POOP IN THE CHIP AISLE.
VIOLATIONS AND CORRECTIVE ACTIONS
Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7;
113700.All violations must be corrected within specified timeframe. Violations that are classified as"MAJOR"pose an immediate threat to public health
and have the potential to cause foodborne illness. All major violations must be corrected immediately.Non-compliance may warrant immediate closure of
the food facility.
OVERALL INSPECTION NOTES AND COMMENTS
FOOD ITEM--LOCATION--TEMP°F--COMMENTS
No Temperature Data Collected
NOTES
Upon arrival, I spoke with the manager in regards to the issue. The manager stated that rodent have been an issue but
actions have been taken in place to prevent them from returning. The manager stated that pest control has been servicing the
facility multiple times a month. During my inspection, I observed the bait boxes in the back storage area of the facility but also
observed numerous rodent droppings on the opposite side of the cargo loading area. The droppings appear to be fresh and
older ones are also observed. The facility did contain a hole behind the bailer, which allowed outside pests to enter into the
facility. The hole has been filled but the back area still contains a fence that is broken into due to the high homeless
population in the area. The manager stated that the pest are possibly being introduced to the area due to the homeless, and
with the homeless damaging the back fence, it leaves an opening for the pest to enter into the facility.Advised all rodent
droppingsto be cleaned, area sanitized and place bait traps at every door to avoid rodents from entering into the facility. I also
advised the manager to place sticky traps underneath all fixtures that contain food products, in order to catch any rodent or
pest as it travels from one aisle to the next.
A re-inspection will occur in 7 days, on or after 03/03/2020
Official inspection report emailed.
The person in charge is responsible for ensuring that the above mentioned facility is in compliance with all applicable sections of the California Health and
Safety Code.If a reinspection is required, fees will be assessed at the current hourly rate.
Received by: Name and Title: kimberly streat, store manager
EH Specialist: VICTOR ACEVEDO Phone: (209)468-0337
FA0024561 C00051662 SC544 02/25/2020
EHD 16-23 Rev.06/30/15 Page 1 of 1 Food Program Complaint Inspection Report
1868 E.
