Thornton Rd. Lodi CA 95242 <br />Program: HW <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR -Notice to Comply) <br />52. An emergency coordinator and modified contingency plan information is lacking. There must be at <br />least one emergency coordinator on site or on call to coordinate emergency response measures, and <br />the following information must be posted by a phone: the name and phone number of the emergency <br />coordinator; location of fire extinguishers, spill control equipment, and if present, fire alarm; and the <br />phone number of the fire department, unless the facility has a direct alarm. Employee completed the <br />provided form and posted it next to the facility phone(s). Corrected on site. <br />79. The box of UW lamps was left open. A UW handler shall manage lamps in a way that prevents <br />releases of any UW or component of a UW to the environment. a UW handler shall contain any lamp in <br />a container or package that is structurally sound, adequate to prevent breakage, and compatible with the <br />contents of the lamp. Such a container or package shall remain closed and shall lack evidence of <br />leakage or damage that could cause leakage under reasonably foreseeable conditions. Employee <br />rearranged the UW lamps into the box and closed it.
