SAN J O A Q U I N Environmental Health Department <br /> COUNTY <br /> Small Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: kuly-1-9 <br /> ate: <br /> DONS CYCLE WORKS 540 N GRANT ST, STOCKTON 2019 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS ll,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 608 CCR 66266.130 Failed to properly manage used oil and fuel filters. <br /> OBSERVATIONS: One black 55 gallon steel drum containing drained used oil filters was observed with the incorrect <br /> accumulation start date. The date was corrected to 5/19/19. <br /> REGULATION: Used oil filters shall be drained of free-flowing oil, stored in a closed, rainproof container, labeled as <br /> "drained used oil filters,"and marked with an accumulation start date. <br /> THE LABEL WAS CORRECTED DURING THE INSPECTION. <br /> OBSERVATIONS: Bills of lading for the recycling of used oil filters for the last three years were not found on site. <br /> REGULATION: A copy of each bill of lading must be kept on site for at least three years. <br /> CORRECTIVE ACTION: Immediately locate a copy of all bills of lading for used oil filters for the last three years, <br /> maintain them on site, and submit copies to the EHD. <br /> This is a minor violation. <br /> Overall Inspection Comments: <br /> An inspection checklist was provided to the facility operator on the day of inspection. The EHD has written the <br /> complete report which replaces the initial checklist. <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, within 30 days of receiving the complete inspection report. <br /> NOTES: <br /> The following items were corrected on site during the inspection: <br /> Item 1: No modified contingency plan and no emergency coordinator. <br /> OBSERVATIONS: The facility did not have a modified contingency plan or an emergency coordinator. Item was <br /> corrected on site by by filling out and posting a provided form. <br /> REGULATION: There must be at least one emergency coordinator on site or on call to coordinate emergency <br /> response measures, and the following information must be posted by a phone: the name and phone number of the <br /> emergency coordinator; location of fire extinguishers, spill control equipment, and if present, fire alarm; and the <br /> phone number of the fire department, unless the facility has a direct alarm. <br /> Item 2: Failed to properly label hazardous waste containers. <br /> OBSERVATIONS: <br /> FA0021986 PRO544704 SC001 07/19/2019 <br /> EHD 22-01 Rev.09/27/2018 Page 5 of 6 Small Quantity Hazardous Waste Generator OIR <br /> 1868 E.
