Current financial responsibility documents are required to be submitted annually. Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, and upload the required documents. This is a minor violation. 113 CCR 2638(c), 2638(d) Failure to submit"Monitoring System Certification Form" within 30 days of completion of the test. A monitoring system certification, and leak detector testing were performed on 9/26/17 for the monitoring system certification, on 10/17/17 for the leak detector testing, on 2/1/18 for the vacum . A test report has not been submitted to the EHD or the California Environmental Reporting System (CERS). A copy of the test report must be submitted within 30 days of the test. Immediately provide this report to the EHD. Maintenance and monitoring records for the last three years were not found on site. maintenance records test results for testing conducted on on 9/26/17 for the monitoring system certification, on 10/17/17 for the leak detector testing, on 2/1/18 for the vacuum sensors were not found on site, the manager on site stated that the contractor did not provide those test result to the facility. These records shall be maintained on site for at least three years. Monitoring records include: (1) date and time of all monitoring or sampling; (2) monitoring equipment calibration and maintenance records; (3) results of any visual observations; (4) results of sample analysis performed a lab or in the field; (5) logs of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6) results of inventory readings and reconciliations. Immediately locate and maintain all maintenance and monitoring records for the last three years on site and submit copies to the EHD. This is a minor violation.
