The designated operator failed to document one or more of the following: On the 10/29/18, 11/28/18, 12/28/18, and 1/27/19 designated operator(DO) inspection report under -Section III The DO failed to add a description to all the components marked N/A or N from section VII through XI. -Section V The owner/operator failed to provide explanation of follow-up actions that will be taken for the statement provided by the DO in section III. -Section X The DO failed to mar YES, NO, N/A and failed to provide a date where necessary under the questions"Has the monitoring system certification been completed within the past 12 months?", "Has the spill container testing been completed within the past 12 months?", "Has the tank tightness testing been completed within required timeframes?" and "Has the line tightness testing been completed within the required timeframes?" During the designated operator inspection, the designated operator shall review the alarm history for the previous month, check that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with documentation taken in response to any alarms to the monthly report. Ensure that designated operators performing inspections at least once every 30 days at this facility and are including all of the required information on the reports. This is a minor violation. 305 HSC 25290.1(e)VPH monitoring of the interstitial spaces of the UST system is not maintained. Based on the alarm history and the Designated Operator reports from 10/29/18, 11/28/18, 12/28/18 and 1/27/19 Under-dispenser containment 11/12 has been loosing brine and the facility has been adding brine to address the low liquid alarm. The sensors monitoring the liquid level in the interstitial space shall be maintained so that a breach in the primary or secondary containment is detected before the hazardous substance is released into the environment. Correct immediately by having a properly licensed, trained, and certified contractor replace the failed component with a functional component(LG 113-x listed, if applicable) and obtain a permit within one business day from the EHD. If the failed component can't be replaced immediately, there is a possibility that the 87 product, and 91 product UST system may be red tagged to prevent fuel inputs. This is a minor violation. 314 HSC 25290.1, 25290.2, 25291 Failed to have secondary containment or secondary containment not tight. Based on the alarm history and the Designated Operator reports from 10/29/18, 11/28/18, 12/28/18 and 1/27/19 Under-dispenser containment 11/12 has been loosing brine indicating a leak in the secondary containment. Secondary containment shall be impervious to the liquid and vapor of the substance contained and constructed to prevent structural weakening as a result of contact with any hazardous substances released from the primary containment. Immediately contact a properly licensed, trained, and certified contractor to make repairs to the UST system under permit and inspection of the EHD. This is a minor violation.
