SAN J 0 A Q U I N Environmental Health Department <br /> CC)U :NIY <br /> Service Request Daily Time and Activity Summary from 3/24/2013 to <br /> 3/24/2050 for SR0079417 located at <br /> C-STORE W/76 FUEL ISLAND <br /> Activity Date Recorded by <br /> 02/01/2019 Deficiency letter creation, scanned the letter and ELD and compatibility guidances PE 2303 MANZO <br /> 150 min and emailed to 'Mel Higginbotham' <mhigginbotham@pmdginc.com>; Muniappa SC 031 <br /> Naidu [EH] <Mnaidu@sjcehd.com>with a request for a conference call this <br /> afternoon. <br /> The following outstanding issues were identified: <br /> "1) Permit application page 3 of 8 is missing applicant's signatures, workers <br /> compensation number, contractor's phone number, and fire district information. <br /> Page 3 of 8 of the permit application lists monitoring panel as 790091-001, TLS- <br /> 350 was identified on your drawings. Defender series overfill valve cut sheet was <br /> provided, but OPW flapper valve is listed on your permit application (page 7 of 8), <br /> correct as appropriate and resubmit. <br /> 2) The submitted ELD work plan is not acceptable for the proposed tanks, please <br /> provide a revised ELD work plan. Please refer to the State Water Resources <br /> Control Board letter dated September 14, 2018 for additional information on ELD <br /> testing requirements (attached). <br /> 3) Provide all tank and sump cut sheets, for example Xerxes installation <br /> instructions were provided in your recent submittal in lieu of cut sheets. <br /> 4) OPW flapper valve calculation sheet was provided, it appears that maybe a <br /> Defender Series flapper valve is being proposed, please clarify, submit the <br /> appropriate calculation worksheet <br /> 5) Confirm that the proposed Phil-Tite sump covers (STP and fill sumps) are <br /> VPH compliant. <br /> 6) Please point out your safety factors on the provided buoyancy calculations; <br /> one safety factor for each tank. Also page 2 of 10 of this calculation indicates that <br /> "the deadman slab shall be 36' long x3' deep x12'wide to achieve adequate <br /> weight to resist buoyancy force", please clarify if a slab of this thickness is being <br /> proposed, none was noted on the submitted plans. <br /> 7) Drawing G.0.1 (as well as several other proposed drawings) show that canopy <br /> footing will be touching or will be extremely close to the proposed fuel piping, <br /> please revise and resubmit to reflect appropriate set back distances. <br /> 8) STP sump details(G4.0 and G4.1) do not show any proposed smart sensors <br /> or vacuum communication ports or vacuum hoses. All proposed vacuum supply <br /> and communication components to be installed must be identified on proposed <br /> drawings. <br /> 9) Per your response#17 all piping cross overs will be a minimum of 12 inches, <br /> please confirm that such cross over distance will be maintained through the entire <br /> piping layout. <br /> 10) Drawing G7.1, Fe Petro leak detector is listed under VMI schedule and <br /> Phil-Tite components are listed under OPW schedule, please correct as <br /> appropriate. <br /> 11) Drawing G12.1, all vacuum zone terminations must be shown for all piping <br /> inside under dispenser containments(UDCs), identify all provisions for <br /> communication testing at each vacuum zone termination (UDCs, sumps, vent <br /> sumps, chase piping, etc.), provide a legend for any symbols (like valves) being <br /> utilized. On this drawing several vacuum hoses terminate without any apparent <br /> valves or penetrations they should be going through. Drawing G5.0 doesn't depict <br /> vacuum communication provisions for UDCs or piping. Also, it is EHD's <br /> understanding that Phill-Tite spill containers are proposed, however OPW spill <br /> containers are included on this drawing (OPW spill containers are also shown on <br /> G4.1 and several other drawings), please clarify and correct as appropriate. <br /> 12) Vacuum zone volumes were requested previously, please clarify where they <br /> Page 4 of 8 <br />
