In order for this application review to continue please address each <br /> comment below and provide additional information (three copies each) as needed; all revisions <br /> should be included in clouds or clearly identified using other methods, only revised pertinent <br /> pages should be included with your submittal: <br /> 1) Permit application page 3 of 8 is missing applicant's signatures, workers compensation <br /> number, contractor's phone number, and fire district information. Page 3 of 8 of the <br /> permit application lists monitoring panel as 790091-001, TLS-350 was identified on your <br /> drawings. Defender series overfill valve cut sheet was provided, but OPW flapper valve <br /> is listed on your permit application (page 7 of 8), correct as appropriate and resubmit. <br /> 2) The submitted ELD work plan is not acceptable for the proposed tanks, please provide a <br /> revised ELD work plan. Please refer to the State Water Resources Control Board letter <br /> dated September 14, 2018 for additional information on ELD testing requirements <br /> (attached). <br /> 3) Provide all tank and sump cut sheets, for example Xerxes installation instructions were <br /> provided in your recent submittal in lieu of cut sheets. <br /> 4) OPW flapper valve calculation sheet was provided, it appears that maybe a Defender <br /> Series flapper valve is being proposed, please clarify, submit the appropriate calculation <br /> worksheet <br /> 5) Confirm that the proposed Phil-Tite sump covers (STP and fill sumps) are VPH <br /> compliant. <br /> 6) Please point out your safety factors on the provided buoyancy calculations; one safety <br /> factor for each tank. Also page 2 of 10 of this calculation indicates that "the deadman <br /> slab shall be 36' long x3' deep x12' wide to achieve adequate weight to resist buoyancy <br /> force", please clarify if a slab of this thickness is being proposed, none was noted on the <br /> submitted plans. <br /> 7) Drawing G.0.1 (as well as several other proposed drawings) show that canopy footing will <br /> be touching or will be extremely close to the proposed fuel piping, please revise and <br /> resubmit to reflect appropriate set back distances. <br /> Page I of 2 141 E Harney Ln., Lodi CA SR0079417 <br />
