The Underground Storage Tank Statement of Understanding and Compliance Form was submitted in February 2019 on an outdated form. This form must be uploaded to the California Environmental Reporting System (CERS). Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov and upload the Underground Storage Tank Statement of Understanding and Compliance Form for this facility. This is a minor violation. 201 CCR 2712(b) Failure to maintain monitoring, maintenance, and follow up action records. -The March 2018 designated operator inspection report states Elite IV was on site on 3/2/18 to replace a sensor. No work order was available for this date. -The 2017 monitoring system certification test results were not found on site. These records shall be maintained on site for at least three years. Monitoring records include: (1) date and time of all monitoring or sampling; (2) monitoring equipment calibration and maintenance records; (3) results of any visual observations; (4) results of sample analysis performed a lab or in the field; (5) logs of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6) results of inventory readings and reconciliations. Immediately locate and maintain all missing maintenance and monitoring records for the last three years on site and submit copies to the EHD. This is a minor violation. 204 CCR 2712(1) Current monitoring plan approved by the EHD not found on site. An approved copy of current, approved monitoring plans were not available for inspection. A copy of these plans shall be accessible on site at all times. Immediately locate and retain a copy on site. This is a minor violation. 209 CCR 2716(a-e) Failure to conduct the designated UST operator visual inspection at least once every 30 days. -The designated operator used outdated inspection forms for the visual inspections. -According to Mr. Tilley, tank top sump inspections are being conducted but not documented on the designated operator reports. During the visual inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and shall check all required items on the report. Ensure that designated operators performing visual inspections at this facility are using the correct forms for all visual inspections and documenting all required information. A designated operator inspection was performed on December 19, 2018 and the next inspection was performed on January 21, 2019, 33 days later. The designated operator visual inspections shall be conducted within 30 days or less of the previous inspection. Ensure that all designated operator inspections are performed within the required time frames. This is a minor violation.
