ARCADIS <br /> • Site sketch showing sample location and note that GPS information was documented <br /> • Sampler name(s) <br /> • Date and time of sample collection <br /> • Type of sample (i.e., matrix) <br /> • Type of preservation <br /> • Lot numbers of vendor-supplied sample containers or specialty-grade water <br /> • Field observations and details important to analysis or integrity of samples <br /> (e.g., heavy rains, odors, colors) <br /> • Instrument readings (e.g., photoionization detector [PID]) <br /> • COC form numbers <br /> • Shipping arrangements (by overnight courier delivery company including air bill <br /> number, or laboratory pickup including name of personnel and time of departure) <br /> • Recipient laboratory <br /> 3.7.2 Chain-of-Custody Records <br /> COC records are used to document sample collection and shipment to the laboratory for <br /> analysis. A COC record will accompany each sample shipment to identify the contents <br /> of each shipment and maintain the custodial integrity of the samples. A sample is <br /> considered to be in someone's custody if it is either in someone's physical possession, <br /> in someone's view, locked up, or kept in a secured area that is restricted to authorized <br /> personnel. Until receipt by the laboratory, the custody of the samples will be the <br /> responsibility of the sample collector or courier. <br /> The shipping containers in which samples are stored (usually a sturdy cooler or ice <br /> chest) will be sealed with nylon adhesive tape to secure the samples. <br /> 3.7.3 Photographs <br /> Photographs will be taken as needed at areas of interest on the Site. They will serve to <br /> verify information entered in the field logbook. When a photograph is taken, the <br /> following information will be written in the daily field notes or will be recorded in a <br /> separate field logbook: <br /> • Time, date, location, and, if appropriate, weather conditions <br /> • Description of the subject photographed, including sample identification number <br /> (if appropriate) <br /> • Point-of-view orientation of the photograph (e.g., to the west, to the east-southeast) <br /> wp-PEA-Gordon-Jul10-RV009808.doc: Page 25 <br />
