SANJ OA U I Environmental Health Department <br /> COUNTY <br /> — <br /> Underground Storage Tank Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> SMK CHEVRON 25651 N HWY 99 ACAMPO May 29 2019 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 209 CCR 2716(a-e) Failure to conduct the designated UST operator visual inspection at least once every 30 days. <br /> A designated operator inspection for the following months was performed more than 30 days: <br /> -Conducted 11/23/18, next conducted 12/24/18 (31 days) <br /> -Conducted 1/18/19, next conducted 2/19/19 (32 days) <br /> The designated operator visual inspections shall be conducted within 30 days or less of the previous inspection. <br /> Ensure that all designated operator inspections are performed within the required time frames. <br /> The designated operator failed to document all the alarms from the attached alarm history on the designated operator <br /> inspection reports and failed to check that they were responded to appropriately. The missing alarms include: <br /> -Q3 (87) gross line fail on 4/24/19 <br /> During the inspection, the designated operator shall review the alarm history since the previous inspection, check that <br /> each alarm was documented and responded to appropriately, and attach a copy of the alarm history with <br /> documentation taken in response to any alarms to the report. Ensure that designated operators performing <br /> inspections at this facility are including all of the required information on the reports. <br /> The designated operator failed to check and note all required items on the visual inspection reports. The incorrect and <br /> missing information included the following from the inspection reports: <br /> -Section V was filled out by the designated operator and did not include the corrective actions that will be taken by the <br /> owner/operator. <br /> -Overfill test information was left blank. <br /> -The reports incorrectly mark that all follow up to previously reported issues were completed and list repeat issues in <br /> Section III. <br /> -The reports incorrectly mark that the overfill testing was completed on time. <br /> -Section III incorrectly states no follow up is needed. <br /> During the visual inspection, the designated operator shall check that all required testing and maintenance for the UST <br /> system have been completed and shall check all required items on the report. Ensure that designated operators <br /> performing visual inspections at this facility are including all of the required information on the reports. <br /> This is a minor violation. <br /> 306 CCR 2630(d) Monitoring equipment is not installed or maintained to detect a leak at the earliest opportunity. <br /> The 7/8 UDC sensor was raised approximately 12 inches off the lowest point of the sump. The 11/12 UDC sensor was <br /> raised approximately 2 inches off the lowest point of the sump. Neither were located to detect a leak at the earliest <br /> opportunity. Monitoring equipment shall be maintained to be able to detect a leak at the earliest possible opportunity. <br /> The sensors were relocated to the lowest point of the sump by the service technician.
