Pagel of 3
Mike Infurna [EH]
From: Shortridge, Julie [Jul ie.Shortridge@arcadis-us.com]
Sent: Tuesday, July 19, 2011 5:15 AM
To: Mike Infurna [EH]
Subject: RE: Groundwater Monitoring Well Permit questions
Attachments: Excavation and Sampling Report.pdf
Hello Mike,
Thank you for getting back to me. Yes, this is the PG&E site on Zuckerman Road. I am very sorry that you were
never provided a copy of the final report—there has been a bit of reshuffling of people working on this site
because a couple people left the company, so it must have gotten overlooked during the change. I'm attaching a
copy of the final report for the work done in August; please let me know if you would like us to send a hard copy
as well.
Aaron Costa (our manager for this project) should have sent you the permit application materials late last week
(including both pages of the permit application, workplan, fees, and MFR form), so if you don't have them
already you should be receiving them shortly. We would like to keep Bob Gray of PG&E as the billing party for
the site MFR form, so we just sent one MFR form (also with PG&E listed as the billing party) in for the "permit
MFR".
We have tentatively scheduled with Woodward Drilling to do this work on Tuesday, August 16th
Please let me know if you have any other questions or need more information. Thanks again for all your
assistance with this!
Julie
From: Mike Infurna [EH] [mailto:MInfurna@sjcehd.com]
Sent: Wednesday, July 13, 2011 5:17 PM
To: Shortridge, Julie
Subject: RE: Groundwater Monitoring Well Permit questions
Julie,
you interpreted all the info really well...good job.
remember, our permit application is TWO pages. make sure page two, the "Licensed
Contractor Declaration-LCD" is submitted IF you, the consultant sign on page one. have the
driller sign ONLY the bottom section of page two, next to ""l,
authorize....."
also, you should know I'm old and close to retiring and i forget stuff....like WHAT site are you
talking about?????
was it the PG&E site on Zuckerman??? or the Ag site on Woodwood? or the Army Air field??
if it's the PG&E site, then I have a concern...
An email from Jamey Peterson (Arcadis) on Oct 18, 2010 advised that I/the EHD would get a
report of findings for the work done at the site in APRIL 2010....
7/19/2011
