One, the first one, identifies the SITE info...as in <br /> Responsible party billing and facility info. It's typically sent in as the site becomes a 'site' with <br /> us. We call it the site MFR and it comes with a three hour Pre-pay that sets up an account <br /> with us. There are many possibilities for the "billing party" as a site enters our programs; <br /> landowner, leasee, potential land-purchaser, financial institutions, etc. The EHD bills this entity <br /> identified on the MFR for all NON permit charges. <br /> when a permit application is received, a billing party must also be identified for charges the <br /> permit fees may not be adequate to cover. FYI, ALL of our field inspection time and time to <br /> process/issue the permit come out of the fees paid with the application ($366 + $122 per well <br /> or $122 for all the borings). Sometimes the PERMIT MFR and the SITE MFR have the billing <br /> going to the same entity. ok. but we still need the form with the signature saying that... 99% of <br /> PERMIT MFRs have the consultant take the "3rd Party Billing"..1% are the landowner or the <br /> facility operator (Repsonsible party). <br /> the PERMIT MFR gets destroyed when the permit is finalled off/done. The SITE MFR stays <br /> active as long as the site is active. 99% of the Third party billing entities for the SITE MFR are <br /> the RPs..not the consultant. <br /> see? <br /> as of today, the SITE 3rd Party billing party is "PG&E" out of San Ramon, CA. IF this stays <br /> the same, NO NEW SITE MFR is needed.
