and still I'm waiting for the report ....still....
also, the EHD needs TWO MFRs typically. One, the first one, identifies the SITE info...as in
Responsible party billing and facility info. It's typically sent in as the site becomes a 'site' with
us. We call it the site MFR and it comes with a three hour Pre-pay that sets up an account
with us. There are many possibilities for the "billing party" as a site enters our programs;
landowner, leasee, potential land-purchaser, financial institutions, etc. The EHD bills this entity
identified on the MFR for all NON permit charges.
when a permit application is received, a billing party must also be identified for charges the
permit fees may not be adequate to cover. FYI, ALL of our field inspection time and time to
process/issue the permit come out of the fees paid with the application ($366 + $122 per well
or $122 for all the borings). Sometimes the PERMIT MFR and the SITE MFR have the billing
going to the same entity. ok. but we still need the form with the signature saying that... 99% of
PERMIT MFRs have the consultant take the "3rd Party Billing"..1% are the landowner or the
facility operator (Repsonsible party).
the PERMIT MFR gets destroyed when the permit is finalled off/done. The SITE MFR stays
active as long as the site is active. 99% of the Third party billing entities for the SITE MFR are
the RPs..not the consultant.
see?
as of today, the SITE 3rd Party billing party is "PG&E" out of San Ramon, CA. IF this stays
the same, NO NEW SITE MFR is needed. But if you want to change the SITE 3rd party billing
info to Arcadis, or Santa, or YOU, then send in ANOTHER MFR and I'll process it.
either way, SITE or PERMIT MFR, the actual form is the same..
so, get me the report or NO NEW PERMIT will be issued.....
and get me at least the PERMIT MFR, the LCD, and, if applicable, any NEW SITE MFR if the
billing info is to be changed..
I get cranky when we send bills to the SITE MFR billing party only to have it come back unpaid
with some explanation that 'I'm not the billing party anymore..."
see?
mike
From: Shortridge, Julie [mailto:Julie.Shortridge@arcadis-us.com]
Sent: Tuesday, July 12, 20119:24 AM
To: Mike Infurna [EH]
Subject: Groundwater Monitoring Well Permit questions
Hello Mike,
Thank you for returning my call yesterday about the fees and requirements for submitting a groundwater
monitoring well permit application.
