CALIFORNIA 95685 <br /> 209 267-9111 <br /> CONTRACTOR'S LICENSE NO.236050 <br /> JOB HAZARD ANALYSIS: The original excavation created by the removal of the <br /> underground tanks has been backfilled and paved over. The initial borings <br /> will be done with a truck mounted drill. Temporary barricades and <br /> construction caution tape will be used during the boring operation to keep <br /> unauthorized personnel from the area being tested. <br /> If after test results of soil samples indicate that a contamination exists, <br /> a supplemental plan of the job hazards will be submitted when it is <br /> determined the extent and how the contamination will be mitigated. <br /> Existing underground piping will be located to determine if any conflict <br /> exists as to the proposed boring. Underground Service Alert (USA) at <br /> 1- (800) - 642-2222 will be notified to locate underground lines affected by <br /> this work. If lines are found to conflict with the proposed work, appropriate <br /> measures will be taken to avoid disruption of services. <br /> A potential chemical hazard may exist. The site investigation being proposed <br /> is to determine if the location and use of the removed underground storage <br /> tanks allowed through leakage and/or spillage of the stored fuels contam- <br /> inated the surrounding soils. <br /> JOB HAZARD SUMMARY: This phase of the project is only to assess the site for <br /> possible contamination and to determine the extent of contamination, if any <br /> exists. <br /> With proper regard to safe working procedures, as per OSHA requirements, this <br /> phase of the work' s potential for health or environmental risk is minimal . <br /> If a potential health or environmental health hazard becomes apparent, all <br /> operations during this phase will cease, and an assessment will be made with <br /> appropriate steps taken to mitigate the problem. <br /> EXPOSURE MONITORING PLAN: For the protection of the crew, an organic vapor <br /> detector also will be used to measure the volatile concentrations in the <br /> breathing zone prior to and during the collection of the samples . Total <br /> ionizable hydrocarbon readings in excess of 1 ppmv may necessitate respiratory <br /> protection for the affected crew members. This requirement is included in <br /> the complete field health and safety plan developed for the project prior to <br /> the start of field work. <br /> PERSONAL PROTECTIVE EQUIPMENT AND ENGINEERING CONTROLS - LEVEL D: Basic <br /> safety equipment and apparel shall consist of safety goggles, hard hats, steel <br /> toe shoes, and protective overalls. All onsite equipment ie: drill rig and <br /> trucks, shall have fire extinguishers. At least one of the vehicles shall <br /> have a first-aid kit. <br /> no-The organic vapor detector, either a photovac (TIP) with a photoionization <br /> detector (PID) or a Foxboro flame ionization detector (FID) will be used in <br /> the breathing zone prior to and during the collection of the samples. <br />
