BOX 98 <br /> SUTTER CREEK, CALIFORNIA 95685 <br /> 209 267-9111 <br /> CONTRACTOR'S LICENSE NO.236050 <br /> SITE WORK: The site is a secured facility with limited access to the general <br /> public. The area that the borings will be made is fenced (see site plan <br /> Plate 3) with an entrance opening of ten feet. After the equipment is in <br /> place, temporary barricades and construction caution tape will be used to <br /> block entrance to the site. <br /> On and offsite communication will be provided by public telephone, cellular <br /> phone, and private two-way radio systems. <br /> DECONTAMINATION MEASURES: The borings will be made using a truck mounted <br /> drill rig equipped with 8-inch hollow stem augers. <br /> Provision for personal hygiene will be use of the onsite facility known as <br /> the "Health Club" . This facility is located within fifty feet of the site <br /> investigation. It is equipped with sanitary and washing facilities. <br /> The collection of any contaminated materials will be placed in 55-gallon <br /> drums and disposed of by American Environmental Management Corporation. <br /> GENERAL SAFE WORK PRACTICES: Wolin & Sons, Inc. Safe Practices and Operations <br /> is incorporated as part of the Site Safety Plan. See Appendix A. <br /> Work is to be done only during daylight hours. The drilling equipment shall <br /> be in proper working order with all guards and protective devices in place and <br /> in proper working order. <br /> SANITATION: The Health Club facility located within fifty feet of the proposed <br /> site investigation. The Health Club will allow the use of their facility for <br /> this phase of work. <br /> STANDARD OPERATING PROCEDURES: Wolin & Sons, Inc. Standard Operating Procedures <br /> are covered in our Safe Practices and Operations Code and is incorporated as <br /> part of this plan. See Appendix A. <br /> EMERGENCY RESPONSE: For Medical emergency, immediately call 911 and request <br /> appropriate service. Then notify the appropriate agencies of the nature of the <br /> problem ie: toxic spill, 1-fire department, 2-Public Health Services. For the <br /> Phone numbers and contact individuals, see Appendix B. <br /> The site entrance is located on Estate Drive 1 , 000 feet west of Brainbridge <br /> Place. To reach Estate Drive from Interstate 5 and Hammer Lane: East on Hammer <br /> Lane to Don Avenue, left on Don Avenue to Waudman Way, left on Waudman Way to <br /> Brainbridge Place to Estate Drive, left on Estate Drive, 1 , 000 feet to entrance <br />�on left at O 'Connor Woods Retirement Community. <br />
