The designated operator visual inspections shall be conducted within 30 days or <br /> less of the previous inspection. Ensure that all designated operator inspections are performed within the required time <br /> frames. <br /> 2)The designated operator failed to check and note all required items on the visual inspection reports. <br /> -The"NA"in sections VII through XI were not explained. <br /> -Section V of the inspection reports were missing follow up actions about scheduling Overfill Prevention Equipment <br /> Testing. <br /> -Section VI of the report was missing the date, name, and signature of the UST owner/operator. <br /> -Section VII was left blank on the 10/24/2019 monthly inspection report. <br /> -The designated operator answered the question regarding UDC sensor locations but did not list which UDC were <br /> visually inspected. <br /> -Secondary containment test date was incorrectly stated as 2/22/2017. <br /> -Tank and line tightness testing information was left blank. <br /> -Tank and line tightness test information was left blank. <br /> During the visual inspection,the designated operator shall check that all required testing and maintenance for the UST <br /> system have been completed and shall check all required items on the report. Ensure that designated operators <br /> performing visual inspections at this facility are including all of the required information on the reports. <br /> This is a Class II violation. <br /> 310 CCR 2636(f)(4) Piping is not fail-safe outside UDCs or fails to stop flow of product at dispenser inside UDCs. <br /> According to the service technician,the 91 tank relay was not shutting down the pump and does not meet the <br /> requirements for annual line tightness testing exemption. All pressurized piping with secondary containment shall be <br /> pressure tested at least once every 12 months at a minimum release detection of 0.1 gallons per hour at 150 percent <br /> of the normal operating pressure. Continuous monitoring systems that are fail safe and shut down the pump when a <br /> leak is detected, or optionally stop the flow of product at the dispenser if the leak is in the under dispenser <br /> containment(UDC), shall satisfy the requirement for annual line tightness testing. Immediately schedule an annual <br /> line tightness test and provide 48 hours notification to the EHD. <br /> This is a Class II violation. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by 11/9/2019. <br /> Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate ($152).
