The response plan is not current and/or not approved by the EHD. The response plan must be uploaded to the California Environmental Reporting System (CERS). Immediately log into CERS, upload the completed form, and submit for review by the EHD. Ensure a copy is available for inspection at any time. This is a minor violation. 110 CCR 2637(e), 2637(f) Failed to submit"Secondary Containment Testing Report Form"within 30 days after the test. Secondary containment testing was performed on 3/29/2018. A test report has not been submitted to the EHD or the California Environmental Reporting System (CERS). A copy of the test report must be submitted within 30 days of the test. Immediately provide this report to the EHD. This is a minor violation. 111 CCR 2638, 26410), 2715(f) Failed to test monitoring and leak detection equipment every 12 months. Annual monitoring system certification and leak detector testing were last performed on 04/05/2018 and was due by 04/2018 . Testing has not been completed and is 3 months past due. These tests are required once every 12 months. Immediately schedule these tests and provide 48 hours notification to the EHD. Please be aware that a follow up inspection will be charged at our current hourly rate. This is a Class II violation. 112 HSC 25293 Failure to maintain UST records to enable the UPA to determine whether the UST is in compliance. Maintenance and monitoring records for the last three years were not found on site. These records shall be maintained on site for at least three years. Monitoring records include: (1) date and time of all monitoring or sampling; (2) monitoring equipment calibration and maintenance records; (3) results of any visual observations; (4) results of sample analysis performed a lab or in the field; (5) logs of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6) results of inventory readings and reconciliations. Immediately locate and maintain all maintenance and monitoring records for the last three years on site and submit copies to the EHD. This is a minor violation. 118 CCR 2715(a)(1)(A), 2715(a)(2) Failure to submit the "Underground Storage Tank Statement of Understanding and Compliance Form". The Underground Storage Tank Statement of Understanding and Compliance Form is not current in CERS. This form must be uploaded to the California Environmental Reporting System (CERS). Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov and upload the Underground Storage Tank Statement of Understanding and Compliance Form for this facility.
