The Secondary containment test report found on site included the address as 3201 W. Hammer Ln, Stockton. Immediately locate the correct report and have it on site . A copy of the correct test report must be submitted to EHD. This is a minor violation. 202 CCR 2715(c)(3), 2715(c)(4) Failure to maintain the"Facility Employee Training Certificate"on-site(after October 1, 2018). The designated operator employee training for the past twelve(12) months not documented or current. The designated operator shall train facility employees for which he or she is responsible in the proper operation and maintenance of the UST system once every 12 months. The training shall include, but is not limited to: 1. Operation of the UST system in a manner consistent with the facility's best management practices 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan 3. Employee's role with regard to spills and overfills as specified in the facility's response plan 4. Name of the contact person(s)for emergencies and monitoring equipment alarms Ensure that employees have been trained by the designated operator, maintain the list on site, and submit a copy of the training records to the EHD. This is a minor violation. 204 CCR 2712(1) Current monitoring plan approved by the EHD not found on site. An approved copy of the monitoring plans were not available for inspection. A copy of these plans shall be accessible on site at all times. Immediately locate and retain a copy on site. This is a minor violation. 206 CCR 2712 Failure to comply with UST operating permit conditions. A designated operator visual inspection performed on 11/29/18, 2/25/19 and 3/27/19 were 3 days, 2 days and 2 days respectively past due. The designated operator visual inspections shall be conducted within 30 days or less of the previous inspection. Ensure that all designated operator inspections are performed within the required time frames. This is a minor violation.
