Ensure that designated operators <br /> performing visual inspections at this facility are including all of the required information on the reports. <br /> This is a minor violation. <br /> 211 CCR 2715(c)(2) No employee present that were trained in the operation and maintenance of the UST system by a <br /> DO. <br /> Failure to inform the EHD of designated operator identification or change within 30days. <br /> A new designated operator(D.0) David Turner completed the monthly visual inspection reports and notification was <br /> not provided to EHD within 30 days of the change . Any changes shall be reported to the EHD within 30 days. <br /> Immediately log into the California Environmental Reporting System (CERS) at http-.Hcers.calepa.ca.gov and upload <br /> the notification identifying all the designated operators for this facility. <br /> This is a minor violation. <br /> 212 CCR 2715(f)(2) Unstaffed facility failed to have or implement a training program approved by the EHD. <br /> A new designated operator was hired on [DATE] and notification was not provided to the EHD within 30 days of the <br /> change. Any changes shall be reported to the EHD within 30 days.
