The following is an itemized list of underground storage tank violations that have not <br /> been addressed for AM PM HAMMER/15 FOOD #83113 as of January 29, 2019. <br /> Open violations from May 24,2018 inspection <br /> Violation#211 -Designated operator employee training not performed or log not kept. <br /> The designated operator failed to list the"Hire Date"for Sally Moreno and there is no record of recent training for <br /> Wes Parkinson.The designated operator shall train facility employees for which he or she is responsible in the <br /> proper operation and maintenance of the UST system once every 12 months.The training shall include, but is not <br /> limited to: <br /> 1. Operation of the UST system in a manner consistent with the facility's best management practices <br /> 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br /> 3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br /> 4. Name of the contact person(s)for emergencies and monitoring equipment alarms <br /> Ensure that employees have been trained by the designated operator, maintain the list on site, and submit a copy of <br /> the training records to the EHD. <br /> Violation#301 -UST system is not operated to prevent unauthorized release,including spills and overfills. <br /> Fuel filters were observed outside the under dispenser containment(UDC)in all dispensers.The underground <br /> storage tank system shall be operated to prevent unauthorized releases. Immediately have a properly licensed, <br /> trained, and certified contractor repair or replace failed component under permit and inspection by EHD. <br /> Open violations from May 25,2017 inspection <br /> Violation#115-Failed to inform the EHD of designated operator identification or change within 30 days. <br /> A new designated operator(DO),Shawn Rodriguez, completed the DO monthly inspection reports from May 2016 <br /> through April 2017 and notification was not provided to the EHD within 30 days of the change. Any changes shall be <br /> reported to the EHD within 30 days. Immediately log into the California Environmental Reporting System(CERS)at <br /> hfp://cers.calepa.ca.gov and upload the notification identifying all the designated operators for this facility. <br /> Violation#211 -Designated operator employee training not performed or log not kept. <br /> The designated operator failed to list the"Hire Date"for Sally Moreno and there is no record of recent training for <br /> Wes Parkinson.
