The designated operator failed to document one or more of the following.. On the 10/19/18,11/16/18, 12/14/18, 1/11/19, 2/6/19, 3/6/19, 4/4/19, 5/3/19 and 6/28/19, designated operator(DO) inspection report under -Section I The DO fail to provide CERS ID -Section III The DO failed to add a description to all the components marked N/A or N from section VII through XI. -Section V The owner/operator failed to provide explanation of follow-up actions that will be taken for the statement provided by the DO in section III. -Section VII The DO fail to mark yes or No for the question "Has each follow-up action of section III from the previous inspection been completed appropriately?" DO refers to work orders but the work order was not attached to the DO report as requested under section VIII of the DO report. -Section IX The DO failed to mark Y/N/NA under the question "Is the under-dispenser containment free of damage, water, debris, and hazardous substance?"for DSL M The DO failed to mark Yes, No or N/A to the question "Are all sensors in under-dispenser containment located to detect a leak at the earliest opportunity?" -Section XI The DO failed to mark Yes or No to the question "Have all individuals performing facility employee duties received the required facility employee training within the past 12 months?" Furthermore, the DO failed to check that Overfill alarms on T2- Diesel 2 Middle on 9/12/18, 9/11/18 9/10/18, T3- Biodiesel Prove out alarm on 10/17/18 and S2- Diesel 2 Middle No Vac alarm on 12/22/18 were responded to appropriately. The Designated operator failed to put the correct date on the November Designated operator report. He wrote 11/19/18, however the alarm history and the signature from the owner/operator state 11/16/19. During the designated operator inspection, the designated operator shall review the alarm history for the previous month, check that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with documentation taken in response to any alarms to the monthly report. Ensure that designated operators performing inspections at least once every 30 days at this facility and are including all of the required information on the reports.
