On June 14, 2007, Mr. von Flue received a phone call from Mr. Grafius. Mr. von Flue went <br />over the documentation that was required from the inspection report. Mr. Grafius stated that <br />while they were cleaning out the small dumpster that the used florescent tubes were found in, <br />they also found some large ballasts that they think came from the large sign on the corner of <br />the property adjacent to the gas station. Mr. Grafius stated that they are holding the ballasts <br />onsite until the property management company can dispose of them. <br />On June 19, 2007, Mr. von Flue received the corrective actions statement and supporting <br />documentation from Mr. Grafius (Attachment 3). <br />Attachments 4, 5, 6 and 7 include parcel information for the property owner, parcel <br />information for the surrounding property, facility and owner information, and the <br />hazardous waste facility file narrative, respectively. <br />Costco Wholesale #658,3250 W. Grant Line Rd., Tracy, CA <br />On September 16, 2003, Mr. Steven Shih, Sr. REHS, EHD, met Mr. Leo Bobadilla, <br />Supervisor, Costco Wholesale, and asked for consent to perform a routine hazardous waste <br />inspection, take photographs, and collect samples, if necessary, at the facility referenced <br />above. Mr. Bobadilla granted consent and accompanied Mr. Shih for the inspection. At the <br />time of inspection, FM 186 was being used to treat spent fuel filters, which were then <br />disposed of in the trash. A valid Environmental Protection Agency (EPA) Identification <br />Number to manage hazardous waste had not been obtained and two hazardous waste <br />containers were not labeled. An inspection report was left with Mr. Bobadilla <br />(Attachment 8). Photos were taken of some of the violations (Attachment 9). <br />On October 15, 2003, Mr. Shih received the return to compliance document and <br />supporting documentation for the September 16, 2003 hazardous waste inspection. <br />On September 20, 2007, Ms. Michelle Henry, Sr. REHS, EHD, performed a routine <br />hazardous waste inspection. Mr. Robert Mendoza, the manager at the fuel station, gave <br />consent to do the routine inspection, take samples if necessary, and to take photos. Mr. <br />Rick Malfatti, Merchandising Manager, also gave consent to do the routine inspection, <br />take samples if necessary, and to take photos. <br />At the fuel station Ms. Henry asked Mr. Mendoza how the facility managed fuel spills. <br />Mr. Mendoza stated employees use FM 186 to clean up the spill and then discard the <br />waste in the trash. One 55 -gallon drum was found improperly labeled. A blue manifest <br />copy (23695030) was found on site from 2005. Two Texas manifests (01261798 & <br />01334208) were found on site and had not been submitted to DTSC. Mr. Mendoza put <br />copies of all three manifests in one envelope to be mailed to DTSC. <br />At the main store, Ms. Henry asked Mr. Malfatti how they managed used fluorescent <br />tubes, non -empty aerosol cans and alkaline batteries. He stated they are thrown away in <br />the trash because Costco has not provided him with a written policy regarding disposal of <br />these items.
