Violation#108 Failed to maintain a hazardous waste contingency plan onsite and submit to required <br /> agencies. <br /> The City has completed and implemented a new contingency plan, and a copy is attached and is located <br /> inside the operations building. <br /> Violation#110 Failed to completed training on hazardous waste management and emergency <br /> response procedures. <br /> All employees have been trained to handle hazardous waste. A copy of all training records and job <br /> descriptions of employees who handle hazardous waste are attached. <br /> Violation#111 Failed to maintain complete personnel training records. <br /> The City does have training records and is kept on site with the Operations Superintendent inside the <br /> operations building. Attached are job descriptions and current training records for employees who <br /> handle hazardous waste. <br /> Violation#115 Failed to keep signed copy of manifests from the designated facility for three years. <br /> Attached is a copy of manifest 004836601FLE. <br /> Violation#116 Failed to comply with uniform hazardous waste manifest exception requirements. <br /> Attached is a copy of manifest 004836601FLE. <br /> Violation#202 Disposed of caused disposal of HW at an unauthorized point. <br /> On May 11, 2016 (2) 20-yard containers were delivered to the LAWTF property. All of the filter cake <br /> product that was on the ground was picked up and placed in these containers by using a tractor. These <br /> (2) containers remained on site with the hazardous waste until May 25, 2016. On May 25, 2016 Clean <br /> Harbor arrived on site and all of the filter cake product from the (2) 20-yard containers was transferred <br /> to the Clean Harbor Transportation bins. These Clean Harbor Transportation bins were taken to a <br /> hazardous waste disposal site. Attached are photo's identifying the area has been cleaned up as well as <br /> a copy of the manifest identifying the hazardous waste was properly disposed on May 25, 2016. The <br /> hazardous waste residue, which was left in the tractor was rinsed and emptied into the hazardous waste <br /> storage containers. <br /> The City is no longer storing this hazardous waste (filter cake) on any surface on this or any other <br /> property. The City has entered into an agreement with Clean Harbors, where Clean Harbors now has a <br /> 20-yard storage bin on site at all times. When hazardous waste is generated the hazardous waste is <br /> immediately placed and stored in the Clean Harbors 20-yard waste bin. This bin is specifically designed <br /> to store and transport hazardous waste, which is lined and covered at all times. When the hazardous <br /> waste container is full or prior to 90-days, Clean Harbors hauls the hazardous waste container to a <br /> hazardous waste facility and replaces the storage bin with an empty storage bin at the same time. <br /> Violation#204 Failed to obtain a permit to treat hazardous waste. <br /> The City is currently applying to obtain a hazardous waste permit. <br />
