Current financial responsibility documents are required to be submitted annually. Immediately log into the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/, and upload the required documents. 2)An approved copy of the financial documents were not available for inspection. A copy of these documents shall be accessible on site at all times. Immediately locate and retain a copy on site. This is a minor violation. 104 CCR 2711 UST operating permit application for facility and tank information not submitted or current. UST Tank Information forms for the 87 and 91 tanks are not current in CERS. The Tank forms for the 87 and 91 tanks incorrectly list the riser as"fiberglass".Any change of information must be updated in CERS within 30 days of the changes. Immediately log into CERS, update the required information, and submit for review by the EHD. This is a Class II violation. 105 CCR 2632(d)(1), 2634(d)Failed to have an approved UST monitoring plan. The monitoring plan is not current and/or not approved by the EHD. The monitoring plans for all tanks incorrectly list the piping secondary containment as"dry".The 91 monitoring plan lists the UDC panel as a TLS350R;the 87 and diesel monitoring plans list the UDC panel as a TLS350.The monitoring plan must be uploaded to the California Environmental Reporting System (CERS). Immediately log into CERS, make the necessary changes, and submit for review by the EHD. This is a minor violation. 107 CCR 2632(d), 2641(h),2711(a) Plot plan/site map not submitted or failed to completely show where monitoring is performed. An accurate UST Monitoring Site Plan was not submitted. The monitoring site plan does not include piping and the legend does not match the symbols used.A site plan must be submitted identifying the locations where monitoring will be performed. Immediately log into the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/and upload a copy of the UST Monitoring Site Plan. This is a minor violation. 113 CCR 2638(c), 2638(d) Failure to submit"Monitoring System Certification Form"within 30 days of completion of the test. Annual monitoring system certification, leak detector testing, and spill container testing was performed on 7/10/2018 and a test report was submitted on 12/12/2018,4 months late. A copy of the test report must be submitted within 30 days of the tests. Take all necessary precautions to ensure testing and submittal of test reports are performed in a timely manner.
