The DO report was initially signed by the <br /> designated operator within the 48 hours however the operator did not sing the report until 2/8/19. Ensure that the <br /> operator is signing the document within the require time frame provided by the regulations. <br /> During the monthly inspection, the designated operator shall review the alarm history for the previous month, check <br /> that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with <br /> documentation taken in response to any alarms to the monthly report. Ensure that designated operators performing <br /> inspections at least once every 30 days at this facility and are including all of the required information on the reports. <br /> This is a minor violation. <br /> 306 CCR 2630(d), 2641(a) Monitoring equipment is not installed or maintained to detect a leak at the earliest <br /> opportunity. <br /> The site has flexible primary and secondary piping for the 87 product, diesel product and 91 product. Jumper hoses <br /> were observed in all product in all UDCs. Any potential release from the primary containment into the secondary <br /> containment in the 87 product, diesel product and 91 product piping does not flow into a UDC collection sump to <br /> detect a leak at the earliest opportunity. Immediately contact a properly licensed, trained, and certified contractor to <br /> ensure that any releases in the 87 product, diesel product and 91 product piping flows into the nearest sump for a <br /> leak to be detected at the earliest possible opportunity. Ensure that all monitoring equipment is maintained to detect <br /> a leak at the earliest opportunity. <br /> This is a Class II violation. <br /> 319 CCR 2635, 2637.1, 2665, 2712 No spill bucket, failed to contain 5 gallons, no means to keep it empty, or failed to <br /> be tested. <br /> The 91 Product spill container failed to have a minimum capacity of five gallons. All spill containers shall have a <br /> minimum capacity of five gallons.
