No work order was found for this leak. <br /> These records shall be maintained on site for at least three years. Monitoring records include: (1) date and time of all <br /> monitoring or sampling; (2) monitoring equipment calibration and maintenance records; (3) results of any visual <br /> observations; (4) results of sample analysis performed a lab or in the field; (5) logs of all readings of gauges or other <br /> monitoring equipment, ground water elevations, or other test results; (6) results of inventory readings and <br /> reconciliations. Immediately locate and maintain all missing maintenance and monitoring records for the last three <br /> years on site and submit copies to the EHD. <br /> This is a repeat violation, Class II. <br /> 204 CCR 2712(1) Current monitoring plan approved by the EHD not found on site. <br /> An approved copy of the monitoring plans were not available for inspection. A copy of these plans shall be accessible <br /> on site at all times. Immediately locate and retain a copy on site. <br /> This is a repeat violation, Class II. <br /> 205 CCR 2712(1), 2634(e) Failure to submit a current UST Response Plan and have it available onsite. <br /> An approved copy of the response plan was not available for inspection. A copy of these plans shall be accessible on <br /> site at all times. Immediately locate and retain a copy on site. <br /> This is a repeat violation, Class II. <br /> 207 HSC 25294 Failed to record unauthorized release from primary containment. <br /> The 1/21/19 designated operator inspection report stated a fuel release was found in dispensers 17, 19, and 20 from a <br /> piping joint. This release was not recorded. Any release from the primary containment that does not escape the <br /> secondary containment and was cleaned up within eight hours of being detected shall be recorded with the following <br /> information: <br /> 1. Operator's name and phone number <br /> 2. List of types, quantities, and concentrations of hazardous substances released <br /> 3. Description of actions taken to control and clean the release <br /> 4. Method and location of disposal of the released substances <br /> 5. Description of actions taken to repair and prevent future releases <br /> 6. Description of method used to reactivate interstitial monitoring after repair or replacement of primary <br /> containment <br /> Immediately record this release with the information required, and maintain it on site with the facility's monitoring <br /> records.
