SANJ OA U I Environmental Health Department <br /> COUNTY <br /> — <br /> Underground Storage Tank Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> 76 EXPRESS TIGER NO 1 5777 S FRENCH CAMP RD STOCKTON January 15 2019 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 209 CCR 2716(a-e) Failure to conduct the designated UST operator visual inspection at least once every 30 days. <br /> The designated operator failed to document all the alarms from the attached alarm history on the designated operator <br /> inspection reports and failed to check that they were responded to appropriately. The missing alarms include: <br /> -1_5 (diesel STP sump)fuel alarms on 2/26/18, 2/25/19, and 3/2/19 <br /> -Sensor out and fuel alarms on all sensors on 7/30/18. <br /> During the monthly inspection, the designated operator shall review the alarm history for the previous month, check <br /> that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with <br /> documentation taken in response to any alarms to the monthly report. Ensure that designated operators performing <br /> monthly inspections at this facility are including all of the required information on the reports. <br /> -A designated operator inspection was performed on 10/19/18 and the next inspection was performed on 11/19/18, 31 <br /> days later. <br /> -A designated operator inspection should have been performed by 11/18/18 and the next inspection was performed on <br /> 12/19/18, 31 days later. <br /> -A designated operator inspection should have been performed by 12/18/18 and the next inspection was performed on <br /> 1/21/9, 34 days later. <br /> -A designated operator inspection should have been performed by 1/17/19 and the next inspection was performed on <br /> 2/21/19, 35 days later. <br /> -A designated operator inspection should have been performed by 2/16/19 and the next inspection was performed on <br /> 3/21/19, 33 days later. <br /> -A designated operator inspection should have been performed by 3/18/19 and the next inspection was performed on <br /> 4/19/19, 32 days later. <br /> The designated operator visual inspections shall be conducted within 30 days or less of the previous inspection. <br /> Ensure that all designated operator inspections are performed within the required time frames. <br /> The designated operator failed to visually inspect the diesel STP sump for leaks, water, or debris during the monthly <br /> inspection following the 2/26/18 and 3/6/18 fuel alarms. During the monthly inspection, the designated operator shall <br /> visually inspect all spill containers and UDC sumps, and any tank top sump that had an alarm for which there is no <br /> record of a service visit. Ensure that designated operators performing inspections at this facility are inspecting all of <br /> the required areas and noting it on the on the reports. <br /> The designated operator failed to check and note all required items on the visual inspection reports. The incorrect and <br /> missing information included the following from the inspection reports: <br /> -CERS ID was left blank. <br /> -Section V was left blank. <br /> -The reports are unsigned by the owner/operator. <br /> -Tank and line integrity testing were left blank. <br /> -The reports include incorrect dispenser numbers. <br /> -The reports include 12 dispensers; the site has 13 dispensers. <br /> During the visual inspection, the designated operator shall check that all required testing and maintenance for the UST <br /> system have been completed and shall check all required items on the report.
