Training for employees on site was last done in February and March <br /> 2018. The designated operator shall train facility employees for which he or she is responsible in the proper operation <br /> and maintenance of the UST system once every 12 months. The training shall include, but is not limited to: <br /> 1. Operation of the UST system in a manner consistent with the facility's best management practices <br /> 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br /> 3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br /> 4. Name of the contact person(s)for emergencies and monitoring equipment alarms <br /> Ensure that employees have been trained by the designated operator, maintain the list on site, and submit a copy of <br /> the training records to the EHD. <br /> This is a minor violation. <br /> 306 CCR 2630(d) Monitoring equipment is not installed or maintained to detect a leak at the earliest opportunity. <br /> DEF leaks were observed in UDCs 15/16 and 17/18. The DEF has crystallized and accumulated on the piping, UDCs, <br /> and the floats, and the floats are not being maintained to detect a leak at the earliest opportunity. Monitoring <br /> equipment shall be maintained to be able to detect a leak at the earliest possible opportunity. Immediately clean all <br /> DEF crystallization from the UDCs and ensure float and chain areas are maintained clean and unobstructed. Ensure <br /> that all monitoring equipment is maintained to detect a leak at the earliest opportunity. <br /> This is a Class II violation. <br /> 320 CCR 2635, 2637.2, 2665, 2712 Failure to comply with the installation, inspection, or record keeping of the overfill <br /> prevention. <br /> Overfill prevention testing was conducted on 1/15/19. This testing was required to be conducted by October 13, 2018. <br /> Ensure all future testing is conducted within required time frames. <br /> This is a Class II violation. <br /> Overall Inspection Comments: <br /> The monitoring system certification was conducted on 1/15/19 and was not witnessed by EHD. According to the <br /> technician on site and submitted test results, all tested items passed. The STP sumps were inspected by EHD <br /> on 4/24/19 during the triennial secondary containment testing.
