The following is an itemized list of underground storage tank violations <br /> been addressed for LODI OIL INC ARCO #183680 as of May 06, 2620 ave not <br /> Open violations from May 09, 2019 inspection <br /> Violation #211 -No employee present that were trained in the operation and maintenance of the UST <br /> by a DO. system <br /> The designated operator employee training for Gagan Mall, the employee on site, was not performed. The <br /> designated operator shall train facility employees for which he or she is responsible in the proper operation and <br /> maintenance of the UST system once every 12 months. The training shall include, but is not limited to: <br /> 1. Operation of the UST system in a manner consistent with the facility's best management practices <br /> 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br /> 3. Employee's role with regard to spills and overfills as specified In the facility's response plan <br /> 4. Name of the contact person(s) for emergencies and monitoring equipment alarms <br /> Ensure that employees have been trained by the designated operator, maintain the list on site, and submit a copy of <br /> the training records to the EHD. <br /> I - <br /> R This violation was corrected ❑ This violation will be corrected by(date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: L� <br /> ti wQ d <br /> Violation#301 -UST system is not operated to prevent unauthorized release,including spills and overfills. <br /> The fuel filters under the dispensers are not located to allow drips and leaks to drain back into the UDC. The <br /> underground storage tank system shall be operated to prevent unauthorized releases. Immediately provide <br /> approved secondary containment for the filters under permit and inspection by the EHD. <br /> This violation was corrected [phis violation will be corrected by(date): �e 9 <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: Pi <br /> Y <br /> ` Violation#302-UST system incapable of detecting leak from primary into secondary containment(pre-Jul <br /> 2003). A X <br /> -The training records prior to October 2018 did not include hire dates. <br /> -The training records included only single names, not first and last names. <br /> All site employees must be trained prior to taking responsibility as a facility employee if after October 1,2018, or <br /> within 30 days of hire if prior to October 1, 2018. The Facility Employee Training Certificate shall contain accurate <br /> information for all employees trained. Prior to October 2018,the list shall include the dates of training and hire <br /> dates for all employees.
