Financial responsibility documents have not been submitted to the EHD. The most recent financial responsibility documents onsite and on CERS are dated 3/28/2019. Current financial responsibility documents are required to be submitted annually. Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, and upload the required documents. This is a repeat violation, Class II. 113 CCR 2638(c), 2638(d) Failure to submit "Monitoring System Certification Form"within 30 days of completion of the test. Annual monitoring system certification, leak detector testing, and spill container testing was performed on 3/18/2018 and a test report was submitted on 8/27/2018, more than 4 months late. A copy of the test report must be submitted within 30 days of the tests. Take all necessary precautions to ensure testing and submittal of test reports are performed in a timely manner. This is a Class II violation. 201 CCR 2712(b) Failure to maintain monitoring, maintenance, and follow up action records. Complete maintenance and monitoring records for the last three years were not found on site. -2018 monitoring system certification test report was not found on site. -According to the DO'S inspection reports, overfill prevention equipment inspection testing was conducted on 3/27/2019 but results were not found on site. These records shall be maintained on site for at least three years. Monitoring records include: (1)date and time of all monitoring or sampling; (2) monitoring equipment calibration and maintenance records; (3) results of any visual observations; (4) results of sample analysis performed a lab or in the field; (5) logs of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6) results of inventory readings and reconciliations. Immediately locate and maintain all maintenance and monitoring records for the last three years on site and submit copies to the EHD. This is a Class II violation. 202 CCR 2715(c)(3), 2715(c)(4) Failure to maintain the "Facility Employee Training Certificate" on-site (after October 1, 2018). The Facility Employee Training Certificate dated 4/19/19 was missing dates for the following sections: -initial training -assuming responsibility as a facility employee All site employees must be trained prior to taking responsibility as a facility employee if after October 1, 2018, or within 30 days of hire if prior to October 1, 2018. The Facility Employee Training Certificate shall contain accurate information for all employees trained. Update the Facility Employee Training Certificate to contain the accurate information and submit a copy of the updated form to the EHD. This is a Class II violation.
