Continue to ensure that <br /> all used oil filters are managed in this way. <br /> 27. Containers of HW fored oil and oily water were left open. All hazardous waste containers shall be <br /> closed at all times except;hen adding or removing waste. Ray Simmons closed each of the HW drums. <br /> Continue to ensure all hazardous waste containers are closed when not adding or removing waste. <br /> 52. An emergency coordinator and modified contingency plan information is lacking. There must be at <br /> least one emergency coordinator on site or on call to coordinate emergency response measures, and <br /> the following information ust be posted by a phone: the name and phone number of the emergency <br /> coordinator; location of fire extinguishers, spill control equipment, and if present, fire alarm; and the <br /> phone number of the firedepartment, unless the facility has a direct alarm. The UW lamps and elE ctronics were not labeled. A UW handler shall label or mark UW to identify the <br /> type of UW. Ray Simmonsmarked the UW as UW lamps and UW electronics. Continue to ensure that <br /> the UW is properly labeled. <br /> 84. The UW lamps and el ctronics did not have documentation that indicated when the UW became a <br /> waste. A UW handler shat be able to demonstrate the length of time that the UW has been accumulated <br /> from the date it became a waste or was received. Ray Simmons marked the UW with an accumulation <br /> start date. Continue to demonstrate the length of time that the UW has been accumulated from the date <br /> it became a waste or was eceived. <br /> Business opened in 1-1-1 . HW & UW have not yet been disposed of. Disposal records for HW, UW, <br /> spent lead acid batteries, and drained used oil filters must be maintained for three years. This facility will <br /> be added on as a small hazardous waste generator. Annual invoice will be sent for applicable programs/ <br /> annual permit fee. Master file record forms completed.
