Secondary <br /> containment shall be provided for all single-walled UST components. Immediately provide approved secondary <br /> containment for the filters under permit and inspection by the EHD. <br /> This is a minor violation. <br /> 315 HSC 25291(e)Water in secondary containment not removed, analyzed, and properly disposed of (pre-Jul 2003). <br /> Liquid was observed in the 87 tank#4 STP sump, the 87 tank#4 fill sump, and the 91 tank#5 fill sump. If water could <br /> enter into the secondary containment by precipitation or infiltration, it must be removed and disposed of properly. <br /> Immediately remove this liquid, make a hazardous waste determination per Title 22 hazardous waste regulations, and <br /> manage it accordingly. Ensure that the sumps are maintained free of liquid. <br /> This is a minor violation. <br /> 320 CCR 2635, 2637.2, 2665, 2712 Failure to comply with the installation, inspection, or record keeping of the overfill <br /> prevention. <br /> Overfill prevention testing was conducted on December 18, 2018. This testing was required to be conducted by <br /> October 13, 2018. Ensure all future testing is conducted within required time frames. <br /> Overfill prevention testing is incomplete. <br /> -The presence or absence of ball floats was not verified during the 12/18/18 overfill prevention inspection, or during the <br /> 4/23/18 reinspection for the Tank#1 flapper replacement testing. Ball floats must be verified as either not present or <br /> located at higher than the flapper locations to ensure proper functionality of the mechanical overfill prevention flappers <br /> at this facility. Overfill prevention testing was required to be fully completed by October 13, 2018. Immediately <br /> contact a properly licensed, trained, and certified contractor to complete the overfill prevention testing at this facility. If <br /> the testing cannot be completed immediately, there is a possibility that the 87 and 91 UST systems may be red <br /> tagged to prevent fuel inputs. <br /> The overfill prevention test results for 12/18/18 include incorrect and missing information. <br /> -The test results included technician training certification information for Phil-Tite. The overfill flappers tested are <br /> OPW. <br /> -Manufacturer test procedures from OPW were not listed on the test result page and were not submitted with the <br /> results. <br /> Test result forms must be fully and accurately completed and submitted to EHD within 30 days of testing. <br /> Immediately update these test results to include all of the required information, maintain a copy of the updated results <br /> on site, and submit a copy to the EHD. <br /> This is a Class II violation. <br /> Overall Inspection Comments: <br /> FA0012979 PRO517272 SCO01 04/23/2019 <br /> EHD 23-01 Rev.09/28/2018 Page 6 Of 7 Underground Storage Tank Program OIR <br /> 1868 E.
