Training records shall be maintained on site until the closure of the facility and shall include: -Name&job title of each employee -Description of each job title listed,including requisite skill,education,qualifications and duties -Description of training to be given to each position,both initial and as an annual refresher -Records that training was completed for each employee Immediately update the hazardous waste management training for this facility and ensure that all employees complete this training within six months of hire and at least annually thereafter. Submit a copy of the updated training records to the EHD by September 6,2014. 119. Failed to properly prepare manifests for transportation of hazardous waste. Manifest numbers 006445119FLE and 006445117FLE did not include an EPA ID number. In the space for EPA ID number,"CESGQ" was written in. A generator who transports,or offers for transportation,hazardous waste,shall prepare a manifest in accordance with Title 22 regulations, including the generators correct EPA ID number and the correct address information. Immediately make all necessary corrections and notifications with DTSC to ensure that all wastes are properly identified under the correct EPA ID number and addresses. Submit a copy of the notification made to DTSC,to the EHD by September 6,2014. Ensure that all future hazardous waste is handled under the correct EPA ID number and generator address. 121.Failed to send generator manifest copies to DTSC within 30 days. Manifest numbers 005498785FLE,005498707FLE,007226316FLE,006445706FLE,006445119FLE and 006445117FLE were not on record with the Department of Toxic Substances Control(DTSC).According to Brad Moniz and Joe W.Allen Jr.they have not been submitting copies to the DTSC. According to Sherry Aebersold,Stericycle should be submitting the copies for Dollar Tree DC. Within 30 days of each shipment of hazardous waste,a generator shall submit a legible copy of each manifest to the DTSC. Immediately verify the generator copy of the above listed manifests have been submitted on your behalf to the DTSC and ensure that all future manifests are submitted to the DTSC within the required time frame. ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE.
