Twelve large fluorescent light bulbs and twelve light fixture housing units were observed <br /> behind the convenience shop. Light bulbs were located in an unlabeled cardboard box. A generator shall <br /> label or mark universal waste with the words 'Universal Waste' and the type of waste (generating <br /> batteries, mercury containing equipment, lamps/bulbs, electronic devices, CRTs, CRT glass, aerosol <br /> cans). Mr. Angel labeled the wastes, and stated that the lights were changed out in the early January of <br /> 2014, but was unable to provide the appropriate maintenance record. A generator of universal waste <br /> shall accumulate universal waste for no longer than one year from the date the waste was generated. <br /> Locate the pertinent maintenance record and provide to the EHD, or if unable to demonstrate the <br /> amount of time these wastes were stored on site, dispose of them as universal waste and submit a copy <br /> of the disposal record to the EHD by February 22, 2014. <br /> Complete and submit a copy of the Return to Compliance Certification form with a statement of how <br /> each violation was corrected, including supporting documentation, by February 22, 2014. <br /> PROVIDED: <br /> -Return to Compliance Certification Form <br /> -Schedule of Free Classes <br /> NOTES: <br /> -Mr. Angle provided a written statement, which indicated that Universal Lighting services and maintains <br /> all the lighting fixtures at this location. <br /> -It appears that no hazardous waste, with the exception of universal waste, was generated at this facility <br /> since the last inspection, which took place on 1-24-2011, according to Mr. Angle, all testing related <br /> wastes are retained by the contractors.
