The following is an itemized list of hazardous waste violations that have not been <br /> addressed for GILL EXPRESS INC as of September21, 2018. <br /> Open violations from August 17, 2018 inspection <br /> Violation#107 - Failed to prepare and implement a contingency plan. <br /> Facility failed to prepare a contingency plan. The owner or operator shall prepare a contingency plan designed to <br /> minimize hazards to human health or the environment from fires, explosions, or any unplanned release of <br /> hazardous waste to air, soil, or surface water and immediately implement that plan whenever there is a fire, <br /> explosion, or release of hazardous waste which could threaten human health or the environment. Immediately <br /> prepare a contingency plan and submit a copy to the EHD. <br /> Violation#114-Failed to keep signed copy of manifests from the designated facility for three years. <br /> Manifest numbers: <br /> -017468380JJK(July 7, 2017) <br /> -016774301JJK(October 19, 2017) <br /> -017791172JJK(April 17, 2018) <br /> -018233915JJK(February 7, 2018) <br /> were found without a signed off copy from the destination facility. <br /> Manifest number 016766647JJK from January 12, 2017 was not found on site. <br /> Hazardous waste generators shall retain copies of all manifests signed off by the destination facility on site for three <br /> years and have them readily available for review. If the generator did not receive a copy of the manifest with the <br /> handwritten signature of the owner or operator of the facility to which the generator's waste was submitted within 35 <br /> days of the date the waste was accepted by the initial transporter, the generator shall contact the transporter and/or <br /> the owner or operator of the designated facility to determine the status of the waste. If the generator did not receive <br /> a copy of the signed off copy of the manifest within 45 days of the date that the waste was accepted by the initial <br /> transporter, the generator shall submit an Exception Report that includes a legible copy of the missing manifest, and <br /> a cover letter signed by the generator explaining the efforts to locate the hazardous waste and the results of those <br /> efforts. Exception reports shall be submitted to: <br /> DTSC Report Repository <br /> Generator Information Services Section <br /> P.O. Box 806 <br /> Sacramento, CA 95812-0806 <br /> Immediately locate a copy of the missing manifests or prepare and submit an exception report to DTSC. Submit to <br /> the EHD a copy of the signed off copy of the manifests or a copy of the exception report submitted to DTSC. <br /> Page 1 of 7 <br />
