The following is an itemized list of hazardous waste violations that have not been <br /> addressed for GILL EXPRESS INC as of September21, 2018. <br /> Open violations from May 02, 2016 inspection <br /> Violation#118-Failed to maintain complete personnel training records. <br /> Employee training records for the management of hazardous waste were not available for review. Employee <br /> training records shall include: <br /> 1. the job title for each position at the facility related to hazardous waste management, and the name of each <br /> employee filling each job; <br /> 2. written job description for each position listed above; <br /> 3. written description of the type and amount of both introductory and continuing training that will be given to <br /> each person filing positions listed above; <br /> 4. records that document that the training has been given to and completed by facility personnel. <br /> Training records on current employees shall be kept until closure of the facility. Training records on former <br /> employees shall be kept for at least three years from the date the employee last worked at the facility. Ensure that <br /> employee training is properly documented. Submit proof of correction to the EHD. <br /> Violation#123 - Failed to report program data electronically. <br /> A submission to the California Electronic Reporting System (CERS) for the hazardous waste program has not been <br /> made. Beginning January 1, 2013, all businesses are required to submit all new(or any changes to existing) <br /> hazardous waste information online to the CERS at http://cers.calepa.ca.gov. Be sure to include your hazardous <br /> waste activity in the Businesses Activities section in CERS in addition to any other relevant activities and required <br /> fields. <br /> Violation#303 - Failed to maintain all communication or alarm systems, spill control, or decontamination <br /> equipment. <br /> One spill pallet, located next to the outdoor used oil tank, was full of black oily liquid. All facility communications or <br /> alarm systems, fire protection equipment, spill control equipment, and decontamination equipment, where required, <br /> shall be tested and maintained as necessary to assure its proper operation in time of emergency. Immediately <br /> begin this practice. Submit proof of correction to the EHD. <br /> Violation#502 - Failed to comply with new hazardous waste tank system assessment requirements. <br /> Facility does not have a tank assessment for the one used oil tanks (unknown capacity, estimated at 1000 gallon). <br /> The owner or operator of a hazardous waste tank shall obtain a written assessment reviewed and certified by an <br /> independent, qualified, professional engineer, registered in California attesting that the system has sufficient <br /> structural integrity, is acceptable for the transferring, storing, and treating of hazardous waste, and that the tanks <br /> and containment system are suitably designed. Immediately have an assessment prepared for these two tanks. <br /> Submit a schedule showing when this will be accomplished to the EHD. <br /> Violation#507 -Failed to conduct daily inspections of hazardous waste tank system and maintain records <br /> onsite. <br /> Inspections of the hazardous waste tank system was not performed or documented. The following items are <br /> required to be inspected at least daily: <br /> 1. overfill/spill control equipment; <br /> 2. aboveground portions of the tank system to detect corrosion or releases of water; <br /> 3. data gathered from monitoring and leak-detection equipment; <br /> 4. outside areas of the tank system including secondary containment to detect erosion or signs of releases of <br /> hazardous waste; <br /> 5. for uncovered tanks, the level of waste in the tank. <br /> Immediately begin required inspections of the hazardous waste tank system, and document the items inspected. <br /> Submit a copy of the inspection log for the next 30 days to the EHD. <br /> Page 6 of 7 <br />
