All copies of each receipt shall <br /> contain all of the following information: <br /> -The name, address, identification number, contact person, and telephone number of the generator, and the <br /> signature of the generator or the generator's representative; <br /> -The date of the shipment; <br /> -The manifest number; <br /> -The volume or quantity of each waste stream received, its California and RCRA waste codes, the waste stream <br /> type listed in subdivision(c), and its proper shipping description, including the hazardous class and United <br /> Nations/North America(UN/NA)identification number, if applicable; <br /> -The name, address, and identification number of the authorized facility to which the hazardous waste will be <br /> transported; <br /> -The transporter's name, address, and identification number; <br /> -The driver's signature; <br /> -A statement, signed by the generator, certifying that the generator has established a program to reduce the <br /> volume or quantity and toxicity of the hazardous waste to the degree, as determined by the generator,to be <br /> economically practicable. <br /> Ensure that all future receipts for hazardous waste shipments using a consolidated manifesting procedure contain <br /> all required information. <br /> This is a minor violation. <br /> 123 HSC 25185(c)(3) Failed to submit a written response within 30 days of receiving an inspection report. <br /> An inspection was last done on 8/11/15 and the inspection report was emailed on 8/12/15 and an inspection report <br /> was issued identifying information to be submitted to bring this site into compliance. This information was required <br /> to be submitted by 9/11/2015. This information received on 10/5/15, after more than 30 days. An operator that <br /> receives an inspection report shall have 30 days to submit a written response that includes a statement <br /> documenting corrective actions taken or proposing corrective actions which will be taken. Ensure that a written <br /> response documenting corrective actions taken or proposed is submitted within 30 days of receiving an inspection <br /> report. <br /> This is a repeat violation, Class II. <br /> 303 CCR 66265.33 Failed to maintain all communication or alarm systems, spill control, or decontamination equipment. <br /> In the hazardous waste storing area a 55 gallon black steel drum containing oily water was on top of a spill <br /> containment pallet that had standing liquid to the top of the spill container.The eye wash area in the lower bay water <br /> reservoir was half empty.All facility communications or alarm systems,fire protection equipment, spill control <br /> equipment, and decontamination equipment,where required, shall be tested and maintained as necessary to assure <br /> its proper operation in time of emergency.
