SAN _10AQUIN Environmental Health Department <br /> COUNTY <br /> Large Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> JIFFY LUBE#2322 500 E KETTLEMAN LN, LODI April 30, 2018 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 403 CCR 66265.173 Failed to keep hazardous waste containers closed except when adding or removing hazardous <br /> waste. <br /> In the hazardous waste storing waste area there were one 55 gallon black steel drum holding oily solids and one 55 <br /> gallon black steel drum containing oily water were observed with open lids. All hazardous waste containers shall be <br /> closed at all times except when adding or removing waste. Immediately close these containers and ensure all <br /> hazardous waste containers are closed when not adding or removing waste. <br /> This is a repeat violation, Class II. <br /> 502 CCR 66265.192(a) Failed to comply with new hazardous waste tank system assessment requirements. <br /> Facility does not have a tank assessment for the waste antifreeze tank located in the hazardous waste storage area. <br /> A tank is defined as a storage device that is not engineered to be moved when full. The owner or operator of a <br /> hazardous waste tank shall obtain a written assessment reviewed and certified by an independent, qualified, <br /> professional engineer, registered in California attesting that the system has sufficient structural integrity, is <br /> acceptable for the transferring, storing, and treating of hazardous waste, and that the tanks and containment system <br /> are suitably designed. Immediately have an assessment prepared for the approximately 100 gallon waste <br /> antifreeze tank. Submit a schedule showing when this will be accomplished to the EHD. <br /> This is a repeat violation, Class ll. <br /> 503 CCR 66265.192(k)Hazardous waste tank assessment is incomplete. <br /> The hazardous waste tank assessment for the used oil tank is incomplete. It is lacking the signature of management <br /> required on page two of the tank assessment.The tank assessment states engineer recommendations on section <br /> four page nine of the documents but no document proving that the recommendations have been satisfied.The <br /> hazardous waste tank assessment shall include all of the following information: <br /> 1.tank configuration (i.e., horizontal, vertical), material of construction, and gross capacity(in gallons); <br /> 2.design standard(s), if available,according to which the tank and ancillary equipment were or will be <br /> constructed and all of the following information: <br /> A. material of construction; <br /> B. material thickness and the method used to determine the thickness; <br /> C.description of tank system piping(material,diameter); <br /> D.description of any internal and external pumps; and <br /> E. sketch or drawing of tank including dimensions. <br /> 3.documented age of the tank system(if tank was previously used), if available, (otherwise, an estimate of the <br /> age); <br /> 4.description and evaluation of any leak detection equipment; <br /> 5. description and evaluation of any corrosion protection equipment, devices, or material; <br /> 6.description and evaluation of any spill prevention or overfill equipment; <br /> 7. description and evaluation of secondary containment for the tank system including applicable secondary <br /> containment for ancillary equipment as required in subsection 66265.193(0,- <br /> 8. hazardous characteristics of the wastes that have been or will be handled. <br /> Immediately amend the tank system assessment to include all required information and submit a copy of the <br /> amendments to the EHD. <br /> This is a repeat violation, Class II. <br /> FA0010973 PR0514476 SC001 04/30/2018 <br /> EHD 22-02 Rev.10/31/2017 Page 7 of 9 Large Quantity Hazardous Waste Generator OIR <br /> 1868 E.
