If the generator did not receive a copy of the manifest with the handwritten <br /> signature of the owner or operator of the facility to which the generator's waste was submitted within 35 <br /> days of the date the waste was accepted by the initial transporter, the generator shall contact the <br /> transporter and/or the owner or operator of the designated facility to determine the status of the waste. <br /> If the generator did not receive a copy of the signed off copy of the manifest within 45 days of the date <br /> that the waste was accepted by the initial transporter, the generator shall submit an Exception Report <br /> that includes a legible copy of the missing manifest, and a cover letter signed by the generator <br /> explaining the efforts to locate the hazardous waste and the results of those efforts. Exception reports <br /> shall be submitted to: <br /> DTSC Report Repository, Generator Information Services Section <br /> P.O. Box 806, Sacramento, CA 95812-0806 <br /> Immediately locate a copy for each of the missing manifest or prepare and submit an exception report to <br /> DTSC. Submit to the EHD a copy of the signed off copy of the manifest or a copy of the exception <br /> report submitted to DTSC. <br /> Submit to SJC EHD within 30 days (07/14/11): <br /> 1. one copy of the completed "Return to Compliance Certification" form <br /> 2. a written corrective action statement for the above violations <br /> 3. requested/ supporting documents ie. disposal manifest records <br /> Notes: <br /> - In 2010 year, facility generated approximately 10775 gallon (43.1 tons) of used oil, 1710 gallon (6.84 <br /> tons) of used antifreeze, 1.3 tons of manifested waste = 51.24 tons of hazardous waste. <br /> - Provided free training schedule. Recommended courses: Hazardous Waste management, Universal <br /> Waste management, Basic Auto Industry Hazardous Waste management, Spill Prevention Control and <br /> Countermeasure for Aboveground Petroleum Storage Tanks facilities <br /> - Provided free information on management of paint booth filters from DTSC: 1. Hazardous Waste <br /> Management for Auto Body shops, 2. Minimizing Paint Waste, 3. Pollution Prevention & Compliance <br /> Opportunities Checklist <br /> - Hazardous waste tank assessment for waste oil and waste antifreeze last prepared by HMT Inspection <br /> w/ P.E. Nelson Acosta on 12/2008. Tank installed around year 2000.
