OTHER SPILLS OR LEAKS-Notify 911 AND The County Environmental Health Department(209468-3420). <br /> Refer to the response procedures outlined in Section 5 of the facility's SPCC (excerpts attached). <br /> e. EARTHQUAKES- Storage containers,tanks, and piping may leak or become unstable following an earthquake. <br /> Equipment and stored materials located on storage racks may be unstable. Once the building is deemed safe for re- <br /> entry,these areas should be inspected by each of the department managers. <br /> 11. Reporting Releases <br /> a. This business will,upon discovery, immediately report any release or threatened release of a hazardous material to <br /> OES and the County Environmental Health Department, and to the Environmental and Facility Manager(541-341- <br /> 3344). This business will provide all State, City, County,Fire, and Public Health or Safety Personnel and emergency <br /> rescue personnel with access to the facility. <br /> b. Release-is defined as spilling, leaking,pumping,pouring, emitting, emptying,discharging, injecting, escaping, <br /> leaching, dumping, or disposing into the environment,unless permitted or authorized by a regulatory agency. <br /> c. Threatened Releases means a condition creating a substantial probability of harm when the probability and potential <br /> of harm make it reasonably necessary to take immediate action to prevent,reduce, or mitigate damage or persons, <br /> property, or the environment. <br /> 12. Post-Incident Reporting/Recording: <br /> a. The time, date, and details of any hazardous materials incident that requires implementation of this plan shall be noted <br /> in the facility's operating record. <br /> Within 15 days of any hazardous materials emergency incident or threatened hazardous materials emergency incident <br /> that triggers implementation of this plan, a written Emergency Incident Report, including, but not limited to a <br /> description of the incident and the facility's response to the incident, must be submitted to the California <br /> Environmental Protection Agency's Department of Toxic Substances Control, the local CUPA, and the local fire <br /> department's hazardous materials program. The report shall include: <br /> i. Name, address,and telephone number of the facility's owner/operator; <br /> ii. Name, address, and telephone number of the facility; <br /> iii. Date,time, and type of incident (e.g.,fire, explosion, etc); <br /> iv. Name and quantity of material(s)involved; <br /> V. The extent of injuries, if any; <br /> vi. An assessment of actual or potential hazards to human health or the environment,where this is applicable; <br /> vii. Estimated quantity and disposition of recovered material that resulted from the incident; <br /> viii. Cause(es)of the incident; <br /> ix. Actions taken in response to the incident; <br /> X.
