Time In: 2:17 pm <br /> Time Out: 3:06 om <br /> Food Program Complaint Inspection Report <br /> Name of Facility: LA SUPERIOR WATERLOO INC Date: 06/02/2020 <br /> Address: 1530 WATERLOO RD, STOCKTON 95205 <br /> Owner/Operator: Telephone: (209)546-7950 <br /> Program Element: 1600-FOOD PROGRAM Complaint#: C00052009 <br /> DESCRIPTION OF COMPLAINT ALLEGATIONS <br /> The San Joaquin County Environmental Health Department received the following complaint: <br /> EMPLOYEES ARE NOT WEARING MASK OR GLOVES,TOUCHING AND SERVING FOOD WITH THERE BARE HANDS <br /> IN ALL DEPARTMENTS. MEAT MARKET, BAKERY,AND RESTAURANT. RESTROOMS ARE VERY DIRTY, <br /> OVERFLOWING WITH POOP. PLEASE SEND INSPECTOR AS SOON AS POSSIBLE. <br /> VIOLATIONS AND CORRECTIVE ACTIONS <br /> Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br /> 113700.All violations must be corrected within specified timeframe. Violations that are classified as"MAJOR"pose an immediate threat to public health <br /> and have the potential to cause foodborne illness.All major violations must be corrected immediately.Non-compliance may warrant immediate closure of <br /> the food facility. <br /> OVERALL INSPECTION NOTES AND COMMENTS <br /> FOOD ITEM--LOCATION--TEMP°F--COMMENTS <br /> No Temperature Data Collected <br /> NOTES <br /> Upon arrival, I spoke with the store director in regards to the complaint and he stated having no knowledge of anyone <br /> complaining or approaching him on the matter. He continued to state that at the moment the restrooms are only being used by <br /> the associates. I observed the restrooms and they contain general debris from general use but did not observe any feces on <br /> the floor, or overflowing of any of the toilets. I then observed the different departments and noticed all staff wearing gloves and <br /> with a face mask.
