SAN JOAQUIN <br />CCUN r <br />Environmental Health Department <br />AMENDED <br />10/23/2020 <br />Small Quantity Hazardous Waste Generator Inspection Report <br />Facility Name: <br />Facility Address: <br />Date: <br />Safeway 1648 <br />2449 W Kettleman Ln, Lodi <br />October 21, 2020 <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />Item # <br />Remarks <br />101 <br />CCR 66262.12 Managed a hazardous waste without an identification number. <br />OBSERVATION: The following uniform hazardous waste manifests use the inactive EPA ID number, <br />CAL000310253, which became inactive on 8/26/2019, according to the Department of Toxic Substance's Control's <br />(DTSC) Hazardous Waste Tracking System (HWTS): <br />- 020589905JJK (4/21/2020) <br />- 020592833JJK (1/30/2020) <br />- 020590106JJK (11/6/2019) <br />The facility's current active EPA ID number, CAR000227637, became active on 7/22/2020, according to DTSC's <br />HWTS. Thus, the facility did not have an active EPA ID number from 8/26/2019 through 7/21/2020. <br />REGULATION GUIDANCE: A hazardous waste generator shall not treat, store, dispose of, transport or offer for <br />transportation, hazardous waste without having received an Environmental Protection Agency (EPA) Identification <br />(ID) Number. A State EPA ID number is required for generators of less than 100 kg RCRA hazardous waste per <br />month, less than 1 kg of acutely hazardous waste per month or any amount of non-RCRA hazardous waste. ID <br />numbers are unique to a business and site location. <br />CORRECTIVE ACTION: The violation was corrected since the facility's most recent hazardous waste manifest <br />020589657JJK, for the hazardous waste pickup on 8/7/2020, uses the active EPA ID number CAR000227637. No <br />further action is required to correct this violation. <br />This is a Class II violation. <br />303 <br />CCR 66262.34(d)(2) Failed to maintain all communication or alarm systems, spill control, or decontamination <br />equipment. <br />OBSERVATION: The emergency eye wash station located next to the Utility Room did not function when tested. <br />REGULATION GUIDANCE: All facility communications or alarm systems, fire protection equipment, spill control <br />equipment, and decontamination equipment, where required, shall be tested and maintained as necessary to assure <br />its proper operation in time of emergency. <br />CORRECTIVE ACTION: Bring the emergency eye wash into proper working order by repair, replacement or testing. <br />Provide a corrective action statement to the San Joaquin County Environmental Health Department within 30 days. <br />This is a Class II violation. <br />FA0012254 PR0538959 SCO01 10/21/2020 <br />EHD 22-01 Rev. 9/16/2020 Page 4 of 6 Small Quantity Hazardous Waste Generator OIR <br />1868 E.
